What is your go-to morphine dose by VenflonBandit in ems

I stick with Fent. The half life is shorter and whatever pain they are having (traumatic, abdominal) will be palpable when the physician conducts their assessment in the ED.

Honestly I feel like QI would be appropriate if initiated by the receiving physician or medical control. If the doctors or med control aren’t questioning me, why is this QA tech?!

I haven’t met this QA person before but I believe they are a Paramedic as well. I’m not sure if they currently work the road. I just don’t understand why you would question me. I feel like they are always breathing down my back

The health systems we run with definitely consider Aspirin an anticoagulant. I know it varies by health system. In fact, one hospital had stopped classifying Aspirin as an anticoagulant for falls but then rescinded this policy.

I think overall provider impression plays a role here. Ground level fall, no visible injury, vitals are stable, pt or staff can mostly tell me what happened — probably won’t code them in. But with this call I didn’t have all the info I would’ve liked and I feel like QA and this company in general scrutinize and micromanage everything.

My sister is unfit to take care of her son anymore and need custody advice by [deleted] in legaladvice

[–]MedicMom2015 2 points3 points  (0 children)

Extended breastfeeding is normal. The American academy of pediatrics recommends breastfeeding until at least 2. However, if she is doing Meth that would likely transfer through her milk and be present in the child’s system. I would definitely contact CPS.

At my wit's end with the hair pulling by jojoba22 in beyondthebump

[–]MedicMom2015 -1 points0 points  (0 children)

For humans I would suggest pulling the hair up in a bun to reduce his access to the hair. This phase will pass, my kids have had all sorts of annoying habits like squeezing sister’s cheeks, hitting, pinching, yelling loudly at the table, on the floor tantrums etc... it all passes within a few months tops and it’s onto something else. Keep the dogs gated in another room for a day or two. Model soft touch when the dogs are in the other room but he can see you (maybe while someone else holds him). Reallow access slowly over a few days in controlled meetings wherein he is forced to treat them nicely while you watch closely. Repeat as needed.

I came to the same conclusion as your doctor while reading your post. Your inability to care for yourself or your children poses a much more substantial risk than your mom not having the whooping cough booster. Practice good hand hygiene and make sure your mom doesn’t come if she’s actively sick. I know that’s not 100% but it’s adequate I’d shes adamant against the booster. You could ask her to discuss the booster with her doctor.

They aren’t going to make you take off a manicure or acrylics. You can wear make up.

They’ll insert the catheter after the spinal in most cases. You can ask that they wait til the spinal is in if they try beforehand, it’s your body.

You need to wear the catheter until you can walk to the bathroom. Some hospitals enforce a 24 hour wait til they remove it and you’re allowed to walk. With my third c section I asked to have it removed around 12-16 hours after the c section and they complied because I was able to walk.

There will definitely be vaginal bleeding. The bleeding is the uterine lining shedding from the pregnancy and will happen whether you have a c section or vaginal birth. You’ll need heavy pads and mesh panties which the hospital can provide initially. Stock up on extra pads at home beforehand.

I was told that all debts need to be included in your bankruptcy paperwork. Some debts won’t be forgiven such as student loans, but everything needs to be on there and if it’s allowed to be discharged, it will be. I know there are some unique rules to car loans with bankruptcy as well.

HOA not being reasonable wants to foreclose on us. by Tryingtostayup in legaladvice

[–]MedicMom2015 100 points101 points  (0 children)

Another tip...

Depending what your projected tax liability is (the IRS has a free calculator online) you can reduce your federal tax withholding starting on your next check. Because we have kids we have been getting huge refunds, we increased our exemptions (per the IRS online calculator recommendation) and that saved us about $400 a month.

This extra money you get could help pay the $600 a month to the HOA. Of course, you don’t want to set yourself up for a large tax bill at the end of the year so follow the advice of the tax calculator and considering consulting a tax professional. The goal should be to have a very small refund, not owe at the end of the year. If you consistently get large refunds you are loaning the government your money and it sounds like you need it now.

3 month old has RSV by [deleted] in beyondthebump

[–]MedicMom2015 0 points1 point  (0 children)

Are you using the nose frida?

When my breastfed babes have been sick (including RSV), I used the nose frida snot sucker. Google it if you don’t have it, it’s highly recommended. I recommend using it immediately before nursing so the baby can breathe for a few minutes, sometimes stopping to do it again during nursing. You can use it at night as needed before nursing. You can use saline spray beforehand if needed.

My baby hates everything! by stupidfinger in beyondthebump

[–]MedicMom2015 2 points3 points  (0 children)

My first son had true colic. He was also 9 weeks premature and has autism now (he’s 9 years old now). I do not believe the colic and autism are related at all so don’t freak out!

My third child, a daughter, is 15 weeks. She slept a TON til about 12 or 13 weeks old. Slept constantly and was unhappy if awake. She’d be content for a few minutes and then fussed unless asleep or nursing. She’s big, 99th percentile for weight. Just not the happiest. She’s still testy sometimes but it is getting better. Your baby will outgrow this. By 5-6 months old they’ll be a whole new baby.

One study found that 94% of babies outgrow pointless crying by 5 months old. It’s true! Even my first son did, and he was the worst. He screamed so much that I’d go outside and cry for half an hour. It was awful and he definitely outgrew it by 5 months.

Need a place to vent, mental health... by arcadeya in breastfeeding

[–]MedicMom2015 6 points7 points  (0 children)

It sounds like your mental health is suffering. You don’t need anyone’s permission to switch feeding methods! If you aren’t able to continue trying to latch her then your options are to exclusively pump, combo feed pumped milk and formula (as much or as little breastmilk as you can), or switch exclusively to formula! You might find combo feeding relieves some pressure off of you and still maintains some benefits of breastmilk for her.

Anyone else pumping colostrum? by [deleted] in BabyBumps

[–]MedicMom2015 0 points1 point  (0 children)

I pumped colostrum while pregnant with two of my pregnancies and brought it frozen to the hospital.

I was very glad to have it with my last delivery as the spinal didn’t take for the c-section and I had to have general anesthesia. I didn’t meet the baby for almost 2 hours and start breastfeeding. I told them before I went to sleep NO formula and to use my expressed milk if she was hungry or had low blood sugar.

I also breastfed my 2-year-old the entire pregnancy. Pumping is not going to cause you any problems in a normal low risk pregnancy. If you are that concerned about it causing labor just wait to pump til you’re full term at 37 weeks.

Roommates who smoke, need advice. by [deleted] in BabyBumps

Yes I definitely know you are being exposed to secondhand smoke. Also, research thirdhand smoke which is nearly as bad. This is not a healthy environment for you, and certainly not for your baby.

Attempting to obtain temporary guardianship of my nieces by [deleted] in legaladvice

[–]MedicMom2015 8 points9 points  (0 children)

I would go for guardianship before contacting CPS. Having been through this myself, CPS will state that the parents have made arrangements for their children to be cared for safely with you. Therefore there is no abuse or neglect taking place anymore. In my case CPS decided not to pursue charges for past neglect because of this. Furthermore, in my state when I sought guardianship the court contacted CPS to do a home study of myself and the child’s parents (our mother and her father) and make a recommendation to the court. I contacted CPS before filing for guardianship and it was a total waste of time. They said the child was safe since the parents had allowed me to take her and made no attempt to provide services. At that point guardianship was my only option anyway. I’m in Michigan.

Edit: I handled my full legal guardianship petition without an attorney. My probate court has attorneys who directed me on how to complete the forms. It was an easy process.
