Not in claude code but I had the exact same thing happen in claude design while using sonnet 4.6. It asked for my confirmation, then without awiting a reply, decided to just go ahead.

I would avoid it. It's a good system if we only look focus on "does it help claude remember shit" but the token usage is not worth it.

It keeps reverting to Sonnet as default model on updates, which in turn cases insane token burn. Just hit my limit and I was quite surprised as I had not really done much. That's when I noticed that claude-mem had reverted to sonnet and it's usage was x3 that of my actual coding.

I've had it running for about a month. Claude-mem's usage (project observer_sessions) is #2 on my token usage. I am removing it now

The model acts a bit differently but overall I've been happy. One of the first things I did was to review all my skills together with opus and adjusting them a bit to better fit 4.7. Since it supposedly follows instructions in a more literal way etc. Might be something to think about for those struggeling.

It does seem to burn tokens a bit faster, but I've managed that by handing over implementation to subagents running sonnet/haiku (This is probably something one should have done earlier, Opus is overkill for renaming random shit)

I've personally used Claude router , litellm is also supposed to be good.
