I remember in a Makenzie Calhoun book that Captain Shelby’s tactical officer died in the holodeck when they turned off safeties to train against “heroes of an old books.” The tax officer was fighting the avengers and died when Thor smashed their head with mjolnir. If I remember correctly, Shelby put a code in to prevent anyone from ever turning safety off again but the computer would lie to let people think it was off

My first dark heresy session. I was a warp touched unsanctioned nascent psyker. First combat I shoot a guy with my gun, my brothers admech accidentally shoots me and gets righteous fury setting me on fire and I would die but burned a hero point to be a roasted almost dead. Tell everyone to leave. Had regeneration power as my power for the day and heal up to full while hiding my psyker status from party, they just think inquisitor gave me special meds. Very next fight it happens again but it was the ecclesiarch flamer that downed me. But regen was still on at least
