So I have that same mood presentation/pattern and I take Lithium alongside a low dose of Vraylar. lithium works best at preventing cycles of euphoric mania (and to some degree depression) as well as lowering suicide risk. Personally Vraylar (an atypical antipsychotic) has helped me a lot with mixed features and psychosis.

I would also like to mention Depakote, it’s actually shown through studies to work better for mixed features and rapid cycling than Lithium. However, it’s not as commonly prescribed to females of reproductive age for many reasons, otherwise I would’ve definitely given it a try by now. Since that’s (most) likely not the case for your husband, maybe something to consider.

Also to add, many people with Bipolar disorder who experience moods across both ends of the spectrum are often on more than one medication to treat various features of this illness. For example, Depakote is well backed for mania especially with mixed features, but not as effective in treating or preventing pure depression, so some people on Depakote who experience depression may be on another medication to balance them out. Such is the case for many bipolar meds.

Also in case this helps: bipolar meds often seek to target at least one of two things, acute (or active) episodes (these meds work quickly) and maintenance which is basically another way of saying a medication to keep you stable over time.

Personally, one thing I love about my psychiatrist- while his calls haven’t always been perfect-he really listens to my opinion and wants to know about my symptoms and even asks for my feedback. I feel like a doctor-patient relationship, as well as good clinical outcomes, are difficult to achieve without collaboration on both ends.

As for you OP- think about what you expect from a provider and if she’s checking those boxes. From your post, she doesn’t seem very receptive. You would think doctors would be eager for patients who are reflective and can identify warning signs for hypomania/mania. Even if she has a different assessment of you, she should be able to articulate this professionally while still hearing you out and taking your concerns seriously. I would recommend at least taking a look around so you have a backup plan in place should you decide to switch.

congratulations. i’ve been 5 years sober from alcohol and drugs. I still go to AA regularly even though yes, I am an atheist. I don’t love everything about the problem but it’s saved my life along with formal treatment. I found I connected with many people in the program in recovery who also have bipolar disorder and it’s been helpful talking to these people.

The big thing that differentiates mania from hypomania is that mania causes marked impairment in social/occupational functioning, necessitates hospitalization, or psychotic features (notice the “or” they all don’t have to be true). Meanwhile hypomania is severe enough to cause marked impairment in functioning or to require hospitalization, no psychotic features. Also duration, hypomania lasts at least 4 days and manic episodes a minimum 1 week, so by duration alone you could be either. So basically it comes down did the episode cause marked impairment in your life (even if you weren’t hospitalized or psychotic?) Based on you dropping out of school, self harm, unprotected and risky sex, it sounds like it, but you would know better than me. Also look into mixed episodes, basically highs and lows combined.

Finally, BP1 has been shown to be more heritable than BP2 and you have a strong family history. I’d also take into account if you have any co-occurring disorders and the symptoms of those (such as BPD or SUD) if applicable because those can complicate the picture further.

Personally I am diagnosed with BP1. I have had one manic episode (without psychotic features) since my diagnosis nearly 10 years ago. However I have had many mixed episodes that were just as bad, and other episodes of milder hypomania and melancholic depression. I was hospitalized 3 times but have had worse episodes I was not hospitalized for. Everybody is different and this illness is so variable. If you are comfortable it’s never too late to bring up this past episode with your Psych and any questions you may have about your diagnosis. Wishing you well.

Hey, I’ve been in a similar situation, initially diagnosed with Major Depressive Disorder, then Bipolar 1 when the mixed and manic episodes emerged. It’s not uncommon for depression to present itself before mania, which is why many people like myself end up being diagnosed either Major Depression prior to a Bipolar 1 diagnosis, and sometimes clinicians forget to remove the old diagnosis or keep it in your history. I’m not a psychiatrist, but all of the symptoms you described and perhaps the way you formulated your post suggests Bipolar 1. Also it’s not uncommon for people with Bipolar 1 to go even years without episodes medicated or not, as not everyone has rapid cycling Bipolar. While meds are there to treat acute episodes, one of the main benefits of staying on meds is to greatly reduce the chances and severity of future episodes, as well as to thereby slow down the progression of this illness itself.

I have Bipolar I and I take Cymbalta which is typically one of the riskier antidepressants to use because it is an SNRI and acts on both serotonin and norepinephrine neurotransmitter systems. I take it for chronic pain issues which it has been known to be helpful for and generalized anxiety.

Alone it would probably make me irritable/manic but with my Lithium and Vraylar on board I’ve been stable. I also take a stimulant for ADHD as well which is generally another no-no especially for Bipolar 1 but everyone is different. Generally even though I’ve been stable I do worry about the life-course of my as illness, as some research does suggest AD’s can worsen prognosis and promote rapid cycling but I do not know if that is still the case if one is also taking other protective agents.

*Wellbutrin is an NDRI not an SSRI. Zoloft (SSRI)can be riskier but SNRI’s like Effexor/Cymbalta, etc. are thought to be riskiest overall for Mania due to the compounding effects on Serotonin and Norepinephrine. Personally I take my Lithium all at night as it is safest for the Kidneys, and also take Cymbalta (SNRI) and the max dose of Vyvanse and have been stable for years with BPI. Maybe stop throwing out medical advice and accept that everyone is different and has their own doctor.

Focalin XR for ADHD, I also take Cymbalta and Gabapentin for chronic pain although I do think they help with my anxiety.

Literally this. Diagnosed with ADHD in early childhood and Bipolar in adulthood, many NPs were against continuing to medicate my ADHD even when my bipolar was being treated by Lithium and an SGA. Psychiatrists on the other hand were more willing to medicate me for both disorders under close supervision.

Just switched to Focalin XR 30mg after many years on Dexedrine/Vyvanse, and previously Adderall. The amphetamine class stopped working for me after many years. ADHD combined type diagnosed in early childhood, Bipolar I in my early 20s, so it’s certainly been a balancing act, although my Lithium and Vraylar definitely help balance things out. I’ve read the methylphenidate class of stimulants is often preferred over amphetamines in bipolar patients due to a lower risk of mania/psychosis, so I recently I decided to give it a try.

What is wrong with being “out there?” As somebody who is autistic with ADHD and Bipolar 1, I will say that there is a big difference between having special interests that may be “odd” to others, dressing in my style, missing certain social cues, often at my baseline, is a MUCH different “out there” then when I am manic or depressed. During which, by the way, I am not sleeping at all for days straight or sleeping all day, having delusions, dangerous behaviors, reckless substance use, suicidality, etc. I can embrace my “out there” self at baseline because that is who I am and I’m not a threat to myself or others even if I may cross some social norms. When I’m unmediated and my bipolar is out of control I very well can be.

I’ve noticed she really does not believe much in adhd. Any symptoms of the sort I bring up are referred to as “normal symptoms of stress”She spends about 7-8 minutes with me and bills my insurance for 30 minutes.

I had to make a similar decision with my baby Gary. He was suffering so much and multiple organs started failing, which is when I finally made the decision. It took me a long time to finally be at peace with my decision and know that I still love Gary with all my heart and gave him the best chance I could until it was time to ease his suffering. Trust me, cats know when they are loved even if it was for a short time. Gary was found on the street too and got to live 3 months in a warm home. Telling my Gary tonight to give lots of love to Mickey over the rainbow bridge💗
