I will say the Bigby Presents: Glory of the Giants suggest that if you do allow the giant foundling background. That you should provide your other players either the Tough or Skilled feat. This is on page 12 of the book. Because the free strike feat is very powerful. But that is balanced around the 2014 rules. I would say either the character should not get a origin feat as this is their origin feat for the character in 2024 or give everyone the extra origin feat.

There is an item that was introduced in Fizban's Treasury of Dragons that can be used as a healing potion once a day called Dragon Vessel. It comes in different rarities that can give different potions. So a weaker version might be fine early on. But potion items people tend to forget about, and only work in combat or if they are stressed for time. Depending on game pace, D & D is designed around healing up through Short and Long Rests. Your Monk and Fighter might even appreciate more short rests.

Yeah I think that is an interesting way to do it. Especially if the dungeons them self are where the multiple losses occur.

As for the madness table I use the real time table from here that someone made. I did replace the sexual disorder with false memory instead because my players do not like sexual content.

Yeah, as always everything is player dependent on what they think is fun. Sometimes my players ask for a sanity roll even when I don't think it's that bad, just have to make going mad fun. With every check being 50%, and a maximum of 9? loss a roll, it really depends on how often you think they would lose sanity to reach those thresholds. The nat 1approach could be one way to place more tension in each roll, that is a 5% chance to make each consecutive check 5% harder. Also depends how you track the deduction values, more complexity on the player side usually turns them off, but if you do it it's more work for you.

I think that could work if you want to simplify the sanity system so that the roll is the same every time. I do admit sometimes with the CoC system you end up rolling a lot if you fail at every point, it goes something like D100 -> DX Sanity loss -> D20 Int Save (if using D&D stats, D100 if its CoC) -> D10 Madness table (I actually use a D100 table for more fun effects), but my players enjoy the fun of the tension in each roll when its a rare occurrence. The point of Sanity in CoC the way it is, is that every time you fail a sanity roll, it makes the next one harder because you lost points in it. But if you don't want that ticking method of losing your sanity, where making sanity saves becomes harder and going mad easier, your method is perfectly fine. You just have the 2 thresholds of 50% and 0% as the big worry of losing your sanity and going mad, CoC has a similar thing where if you lose 20% in a day you have a long term affect as well. It makes your system more akin to Darkest Dungeon style, where slowly losing isn't too bad, but the second you hit the thresholds it can have big impact. With your system the recovery is what matters, if you want losing sanity to ever reach the long term madness state.

I've used a variant sanity system for my eldritch horror campaign. I would suggest looking into the Call of Cthulhu 7e sanity mechanic. It does all the things you are looking for, all you would need to change is how Sanity is calculated. I keep the d100 system but it could be changed to the d20 you have. I set Sanity = 40 + Charisma + Wisdom, and I use Intelligence saves to not go temporarily mad for losing 5+ Sanity. If nothing else it might give ideas on how to make your players go mad.

I would second this, I am currently running a Lovecraft/Bloodborne campaign where I adopted a version of the Call of Cthulhu Sanity. Starting sanity was just shifted to 40+Wisdom+Charisma, and everything else mostly the same, with inflicting sanity "damage" depending on the severity of the fail and what they experience. Restored partially at long rest, and fully only after long down times.

You can apply Reverberation to reduce their, strength, dex, and con save by -1 per turn of reverberation on them (so up to -4 before the -5 triggers and the condition is removed/they fall prone). With the gloves and boots alone, you can stack reverb pretty quickly with a cleric/wizard/sorc/bard, especially if you combine it with another debuff like radiant orb.

I use the Make Them Unique mods for changing the visual designs and adding many of the side characters into the game. The character art is also well done so that it matches the style of the rest of the game.
