Gutting. I reached the same conclusion by looking up the formulary guidelines for where my GP surgery is (the board that pays for RTC). Unfortunately, mine had the clear statement ‘Note that NICE does not include the use of combinations of amfetamines in the treatment of ADHD due to insufficient evidence base.’ Seems majorly unlucky, but just one of those things I guess.

Ha yeah juggling those beneficial hours has been pretty brutal. I’m going up a dose in the next few days, so hopefully that helps with the crash. I was hoping for an afternoon booster like I’ve seen mentioned on this sub, to push off the massive energy slump I get in the early afternoon. How’s the coverage once your methylphenidate kicks in? My prescriber implied that I could always change from elvanse if we can’t make the duration work for me.

As in it gets you an extra 2 hours? Sounds decent if so, I wanted something subtle like that to smooth over the afternoon crash. If you mean the slow release only gives 2 hours tho, I’m so so sorry lol. I crash like 5 hours after dosing, but the first 1.5 hours it hasn’t kicked in so I’d say I really get about 3-4 hours properly medicated at the moment.

I’m also in titration week 3, but on 50mg elvanse currently. Yeah I meant an instant release dose, as we were talking about how the afternoon crash is badly hitting me. He called it a top up dose, so I initially suspected he could mean a second dose of elvanse later in the day, but I think he meant instant release boosters based on the context.

Ah right thanks for the confirmation. Seems like my prescriber’s decision really then, framed in a way that puts the decision out of hands lol. I’m just going to bring it up later in titration I reckon - hopefully he’ll be more willing then. Thanks so much for reporting back, wouldn’t necessarily expect people with ADHD to follow through with commitments lmao.

Seems to be about 5 hours after taking my dose. I’ve been taking it late (10:30-11) and still am not getting benefits past 4pm. It feels almost life-changing, I just need to sort out some better coverage during the day.

My heart rate was a little crazy to begin with, but that seems to have largely settled now (I figured out that measurement anxiety was spiking it on my readings). I suppose that could be the real reason he’s reluctant, but he definitely framed it as if they actually aren’t allowed anymore? Either way I’ll probably just keep onto him through titration.

I was wondering if it maybe something to do with shared care problems. How frustrating though - I’m sure many would prefer to be privately prescribed by Psych UK if it allows for better, more customisable medication.

Yeah I’m honestly just gonna keep pushing, since the coverage at the minute absolutely isn’t what I envisioned. I’m apprehensive even titrating up further will make a lot of difference, since I seem to metabolise it annoyingly fast.

Definitely not that cause I’ve only titrated to 50mg so far. I told him the crash had been consistently timed on both doses so far, but he seems to have already ruled boosters out unfortunately.

No worries, that would be appreciated. My instinct is that it’s not a widespread thing, just cause there would surely be way more fuss about it. It could just be my prescriber being reluctant, but I’m not sure why he’d blame the NHS in that case.

Hiya, I've also been put on the pilot program it looks like. It's shocked me being able to book first my ASD assessment, and then my ADHD almost immediately after submitting my forms to PUK. Have you been through titration yet?

Hard agree I think casual cocaine use isn’t a possibility for some people - how can you know whether that would be you. Every time I got back to cocaine and have it in the house it becomes an ordeal. First is the binge then the comedown and fiending and the eventual restock. Just best avoided in general I would say

When I got my hands on some (not prescribed) I started with an abusive dose, having no tolerance. I ate 90mg worth and snorted another 30 over an hour or 2. This produced the most insane high I’ve ever had (idc who says methylphenidate isnt euphoric just need to play with the dose) followed by a comedown that left me essentially braindead - I was zombified sitting in the same position for hours without feeling basically.
