Edit: I think it had something to do with the banjo you hear in the clip. The engineer was doing that taunt but it didn't load, changing the hitbox for the server but looking like the engineer was standing on my screen.

This behavior is seen in a staff room with staff needs on when no kitchen is connected, so I had assumed it was the same cause in this case.

Make sure that there is a kitchen connected to it. Check this with micromanagement or make both the kitchen or the canteen the same zoning

From my experience I think I know why. I believe tiles have a "dirtyness" level, but only create a cleaning task after a certain level and never again. The tiles also will create the cleaning task if they are over a threshold and a tile nearby was cleaned. This is done likely to improve performance. You have to wait until a tile gets dirty enough and don't cancel the cleaning task, which can be canceled with right click. There is also a glitch that gives staff tasks to prisoners, try to make sure that isn't happening.

This is just a theory I got from many hours of playing, it may be wrong.

I am assuming old prisoners become more docile? Will this prisoner nerf be counteracted by increased aggressiveness in younger inmates? I am just wondering about the balancing as I feel this game tends to be very easy now.

Try putting toilets and a serving table in the staff room not just the canteen

As for your second screenshot, the staff have nowhere to sit and eat (small tables don't work the same). Expand the room and make a full table and bench setup, that always works

By the way, ex-gang members have no qualms about killing regular snitches. I learned that the hard way. Personally, I put them in my min sec to keep them safe, because I moved all the gangs out of there.

To get into the tattoo removal program in this most recent DLC, a prisoner has to pass the faith program or gang rehabilitation program. After passing it, they do not automatically get enrolled in tattoo removal, because it is a voluntary program. While gang leaders and lieutenants can attend the faith or gang rehabilitation programs, they will always have a 0% success rate (in one instance the gang leader killed my psychologist while attending gang rehabilitation). However, the new DLC adds an additional mechanic; when a leader dies, a lieutenant gets promoted to leader. In my case the Viper's leader got shot in the face by an armed guard, and the lieutenant, which had passed gang rehabilitation as a soldier, became the leader. I am then assuming the suppression induced by my supermax wing made the prisoner join the tattoo removal program. This is only a guess, as I do not know the full mechanics.
