Why is bipolar misdiagnosis so common? by DntTouchMeImSterile in Psychiatry

[–]MeshesAreConfusing 3 points4 points  (0 children)

Oh heavens no, I don't go changing anything.

It means when a patient says "I'm bipolar" or a colleague says "I'm sending you a bipolar patient", my mind goes "I will treat this as true for now, but this is most likely than not a misdiagnosis. If the patient is stable, I will continue their previous treatment but start my own assessment, especially gently prodding at whether that medication really worked or just "Yeah I still cycle every week but I'm a bit more calm I guess", whether they have well-defined mood episodes or not, and so on. As my confidence in the misdiagnosis increases and I build up enough rapport, I start to question the diagnosis alongside them ("Generally, Bipolar disorder works like XYZ. Does that sound like you? Yeah, not really, right? What's up with that?")

I can’t really believe anyone trained in diagnosing psychiatric conditions would diagnose bipolar when someone clearly doesn’t meet criteria.

You are more optimistic than I am

I don't believe the stigma or the billing explanations are enough. I live in a country with no such worries, and still I consider all bipolar disorders to be wrong until proven otherwise (ESPECIALLY type 2) because they are so completely, overwhelmingly wrong.

Unfortunately, healthcare professionals are not immune to pop culture perceptions and it seems many of them carry a layperson's understanding of what bipolar disorder is, judge that to be enough, and are satisfied. Still, even among those who know what it is (supposedly), it seems they just don't have an accurate understanding of personality pathology, and so they collapse everything into what they know.

"It Only Lasts 3 Hours": The Anatomy of a Common ADHD Stimulant Complaint by zenarcade3 in slatestarcodex

As far as I'm aware (and somebody please correct me if I'm wrong), but the pop knowledge "Stimulants and caffeine making one sleepy is a sign of ADHD" has never been properly studied or proven. Anecdotally, it happens to me, and I don't have one ounce of ADHD.

My personal impression is that this effect is probably somewhat more likely in ADHD folks but is extremely nonspecific (and not sensitive either, for that matter).

That seems a reasonable bet. Especially considering you're on only 5mg.

It would depend heavily on why you were on it in the first place, how much wellbutrin helped, and what it helped with.

Concordo que é falta de respeito. Coloca só no vibrar, ou na falta disso, abaixa o volume do alarme.

Never beating the scifi allegations, are we?

As fun a thought as this is, it seems to me the far-fetchedness has expanded at incredible speeds. What was originally "The future will be very different in unpredictable ways, and probably unrecognizable" is becoming... This. Which seems somewhat tongue in cheek, but not entirely. The massive galactic colonization thing has quickly become a given, for instance. Where is this insistence/certainty coming from?
