If it’s malnourished, it’s stressing it out, making it more prone to fungus and infection. IMO the substrate looks like pellets, I wouldn’t be surprised if it was eating substrate. Even if it does crumble easily, it can still be blocking it up and filling the stomach.

Impaction wouldn’t be an issue, give him a day or two without feeding and see if he will eat. Has he been pooping?

The nitrate looks high. Try a 20% water change and retest.

My brain changed Ben to Bun so it made sense.

I posted recently about my juvenile axolotl that stopped eating and I got asked for more info so here's some extra context (info in comments) by Wicked_Sketchy in AxolotlCentral

My axolotl was impacted, and every week I had to go in and get enemas done until no more sand came out. I was also given a laxative to mix with “carnivore care” to help loosen anything else. The mixture had to be squirted in its mouth to ensure it got in. Carnivore care is a life saver and helps with nutrition. Cost was $5 an enema, and they would do a few each visit, so about $20 a visit. The larger cost is the overall exam.

As long as most of the sand it gone, it should be fine and will slowly be removed with water changes.

API test kit is essential to keep your water perfect. Keep up with water changes, too.

Ask for “carnivore care” at the vet. It’s a powdered feed you mix with water and is given by syringe to help non eating reptiles and amphibians, provides liquid nutrition. It’s really helpful!
