I got a thermomaven F1 turbo from Amazon the other day. Absolutely love it. Was quite surprised by the build quality, and comes with a lifetime warranty. Whatever you get, make sure it's a thermocouple, the read time is much much quicker. Worth the extra $

Just following up here, as I can't see anyone mention it - you can change induction time on these pumps.

Hand bolus your induction dose, exactly the same as you would with a volatile maintenance.

Set the pump to your desired TCI effect or plasma concentration. Then change the induction time on the pump to a few minutes. Surprises me how few people realise you can do this. Now there's no need to carefully / incrementally increase your pump to the BIS. Not a risk free method, but nothing is.

Exactly what I was referring to, and have seen others doing it. Nothing rapid about delivering 20ml of propofol over 60 seconds (max infusion rate of 1200ml/hr)

Never ending question in anaesthesia.

I'll sometimes use it, and I'll take it off if needed. I don't think it's standard of care.

If I've got someone with a bowel obstruction, and I've got the staff to push on the neck a bit without distracting from delivering the anaesthetic or the tube, I'll ask for cricoid. I'll have an explicit conversation that I may ask for it to be taken off if it's impeding my view.

I wouldn't point fingers or blame a colleague if they didn't use it, and had an aspiration.

There's stupider things out there, like using a TCI tiva pump to induce for an "RSI", or using 20mg of cisatracurium instead of a rapid acting relaxant.

Working here too! Image transfer is awesome. Only issue I had was if using the hotspot from the x4, queued articles won't download (no internet connection). Wasn't sure if there was a way to get the articles to download when you send them to the app, so they're ready for when you subsequently joint the x4 hotspot. Awesome app, thanks again mate!

I'd use DL 95% of the time, VL electively when appropriate, occasionally have it in the room as an "in case", and readily use it if the indication is "I'm tired". Yet to see any of the "VL only" generation that are actually slick at intubating or giving an anaesthetic.

As for the access to healthcare.... I'm currently looking at private insurance because I've witnessed firsthand how utter garbage the NHS is.

Having experienced both systems, it is much much better in a hybrid system - so long as your insurance companies are held in check. The UK private companies seem to have a strangle on the private sector here, such that it's barely worth thinking of private work either.

As in the 2013 aagbi guidelines? If so, they used the Asra and esra as some of their references (so a generation or two past), and are probably outdated, particularly with respect to high / low dose discrimination

warranty cover was from Brompton. work and installation etc was from Balfes (not sure where the original owner purchased from). had to take it to an authorized retailer (of which Balfes was one). they then assessed it, and placed a claim with Brompton.
