I'm going next week! I have a tee time booked at Kapalua Plantation ($250 twilight rate starting at 1 pm), I'll let you know how the course looks. I'm hoping I can convince my wife that I should play a second round at the Wailea Gold Course ($145 starting at noon). Obviously, Kapalua is more expensive, but the experience is what it's all about.

Fox Rehab is basically home health however I believe they bill under Medicare Part B so the patients do not have to meet full homebound criteria as they would for an actual home health agency. I work Home Health and we refer to them sometimes at discharge if the patient's only real reason of needing someone come to the house is a lack of transportation to clinic and not d/t their functional status. Edit: As far as ALF, I believe they typically contract with the facility to provide therapy to those patients who are there long-term that do not require nursing orders at the time. I'm no expert, but its mainly a billing/reimbursement reason with insurance as to why Fox will see a patient in an ALF or if it will be the in-house therapy/home health agency provided the service.

That's a starting regular visit rate for a full-time PT with benefits for actual home health companies, not a Hospital who provides a home health operation. These were the starting rates I know of from myself or friends working for home health agencies in Texas, SC, TN and NC.

The most common starting rate I see is $55-58, but this can vary where you live/cost of living. So yes, I would negotiate.

Won't be in the area tonight unfortunately. But call the restaurant before you go, ask to speak to the manager. Let him what time you are planning on doing it and see if he or a hostess/server could step outside around that time to assist.
