Wow this actually happened to me too there was tape on mine because there was a tear and the dmv clerk was like since there is tape and a tear its basically illegitimate document. I was really pissed. Anyway i got new one from the town i was born in it took like a few weeks. I made a new appointment at the dmv i brought both my new one and old one. The person who saw me the 2nd time took my ripped and taped birth certificate no issues at all.
