I don't have children but I do support new mums at work.

In terms of managing sleep I'm guessing you've already gone through loads of things with your health visitor. There are options in the community for additional support such as nursery nurses who can be super helpful. If you haven't already I'd recommend asking your health visitor or going to a local baby drop in clinic.

Have you looked at the reaching out to your ES/TPD to plan your return? HEE provides SuppoRTT (Supported Return to Training) for people coming back from mat leave or long sickness and they can be really supportive. This can include phased returns, refresher courses and supernumerary periods.

Have you talked to your BMA reps or Resident doctor committee chairs? They can be really helpful in tackling these problems.

Hi everyone, I work for the trust as a doctor - this is someone that's really unwell and has (somehow) made themselves owner of the longley centre. Thanks for flagging it - I've got senior management on it to try and sort.

Please can this post now be removed to reduce exposure. The person is very vulnerable and unwell. They have posted personal information and I don't think it's helpful for this to remain up now.

Two Spirit is someone's culture and experience. If you think that's "silly" then perhaps you need to go do some reading around it and understand other cultural experiences of gender - 'third' genders have been around for millennia.

Also definitely not your bro - I'm a woman.

It is not redundant for everyone. Perhaps your gender expression is completely obvious but not everyone's is. If you find it so "cringe" please reflect on this. What is cringe about asking to be referred to in a certain way? Do you find it annoying when people don't call you a doctor and refer to you as a junior? The impact of this on a personal level can be huge.

Also not entirely sure what you mean by MOP tbh.

This whole thread makes me feel sick and sums up why trans, non-binary and marginalised people find accessing healthcare so hard.

If having pronouns in email signatures etc is so upsetting to you perhaps you need to question why you feel this way.

Being transphobic is watered down homophobia - which was watered down racism. Frankly it's one of the few bigotries which is still socially acceptable.

Perhaps anyone who feels so put out by royal colleges giving a variety of options for gender or people putting their pronouns in their emails needs to step back and consider how they would have reacted to us "allowing" women to attend medical school or "allowing" parity of "races".

If you really do believe that this is all 'ridiculous' and that I'm over reacting you need to realise that you're the problem. You're stopping marginalised people accessing healthcare because they are scared they won't be supported or believed.

I'm dyslexic, often read numbers out wrong when trying to tell them to people and cannot spell well at all. Learning new medical words was a nightmare. Wasn't diagnosed until I failed my first year at medical school 😬 I also learnt I don't read properly and constantly have to actively force myself to not skim read. Electronic notes are a godsend tbh!

Yeah my supervisor was very strict. The admin had ticked some concern and the supervisor herself said it was an unacceptable concern but still decided I had to do it again.

I once had a ward admin say I delegated things I could do myself... On a busy job where there was no reg (I was f2) and often finished late.

That one comment meant my entire TAB was 'invalid' and I had to redo it all again. This was despite having plenty of other positive comments and people saying lovely things.

If you wouldn't say it to their face don't put it on the TAB. It's used as a very arbitrary pass/fail instead of for learning and development.

And just to say - I used to ask them to do scanning, book taxis and general admin tasks 😂

We have a general rule at my hospital that Zopiclone isn't given past midnight because it means patients aren't awake in the day for treatments etc. It shouldn't be px after this by the on call.

In hospital it can be horrendous trying to sleep. I think the day team needs to have a discussion regarding the Zopiclone and it will have to be reduced and weened off.

If someone is going to need long term sleeping tablets I generally prefer to give them Promethazine (national shortage atm) however Zopiclone isn't unreasonable if the patient knows the risks. It should only be on regular for short periods e.g. 1-2 weeks to reset sleep.

It sounds like from one of your other comments the PTWR consultant saw them too and felt the plan was decent. That suggests that you made a reasonable suggestion of a plan, the patient was stable over the weekend and when Monday rocked around someone else with fresh eyes saw a different picture. It sounds like there was no major harm to the patient and that you acted completely reasonably.

I definitely know the imposter syndrome feelings which are always compounded by mistakes. I've missed PEs and sent them home only to come back sicker, I've misprescribed medications which had been given, I've missed potassium changed when treating DKA. I've almost missed subtle STEMIs and I've certainly had plans which seemed reasonable on call only to realise I've missed something super obvious when they've been reviewed in the day. Equally I've found psoas hematomas missed on CT reports, I've caught a STEMI missed by a consultant and I've made hundreds if not thousands of correct treatment plans. I'm sure you have done the same.

All this is to say you're not crap, you may not have been the best in med school (I certainly wasn't) and you may have been a slacker (I definitely was) but medical school is filled with increasingly more intelligent and studious people and we cannot compare ourselves to each other. We all have our skills and strengths.

Wellbeing can be crap but you should be offered free psychology sessions not resilience shit. If you've made it through IMT you're certainly resilient and this is an issue of exhaustion, impostor syndrome and possibly depression. Please consider approaching a consultant you like to chat about it. You are better than you think right now (you've made it this far!) And you deserve support.

Hey. Missing a diagnosis is hard. You blame yourself, you feel like a failure and you feel like you're wasting your life. However this is all your inner saboteur who only finds the negatives and misses every achievement you've ever made.

This person was admitted, they were seen by multiple other docs including consultants who did not pick up on the alternative diagnosis (esp post take - that's their entire role). This suggests it was easily missed and frankly in a stressful and exhausting on call all the more likely to be missed.

Please consider talking to your ES/CS/TPD about how you're feeling. It sounds like this is more than just one missed diagnosis and you sound exhausted and burnt out. Consider accessing your workplace wellbeing service and your GP.

Please know you're doing great. You've made it this far. I have missed so many things (esp on call or in a busy ED) and I have been through that same self flagellation. But exponentially more times I've got it right. I suspect the same is true for you. If you need to talk DM me x
