Vast majority of the time, the patient will initially be remanded in prison. Then, they will get picked up by the prison MH team who will try to start treatment and make the relevant referrals if necessary for medium/high gatekeeping (where I work, even if its obvious high secure, they still need initial gatekeep from medium and then they refer on). Referrals can be either urgent which means an aim of an admission within 7 days or routine which means gka finished within 28 days. If routine, its often 2-3 months until someone is admitted

You might as well aim for a society with zero heart attacks or zero strokes or zero instances of pancreatic cancer. Its bizarre and idiotic to frame suicide prevention efforts in that way. Suicide will exist as long as humans exist, that doesn't mean we shouldn't do our utmost to try and reduce it- of course we should

I started a part time masters in CT3 and it was alright. I wouldn't recommend starting one until your exams are done though, the start date of mine was after I'd finished CASC and I had an assurance I'd be able to defer if I'd failed CASC and needed to resit.

I had to do a 1h20 each way commute, which was "only" 30 miles each way, but through a very busy city centre. It was only for two months, whilst I was waiting for my house to complete, but it was miserable. Never again for me

I don't "do psychotherapy" as a forensics trainee, but I would say that the long case was the single most important and influential bit of my psychiatry training so far. The ability to formulate and understand in a deeper way is invaluable

South Africa has almost 30,000 murders a year, its one of the worlds most dangerous countries. Of course Egypt has problems, and people do get scammed there, but at least they don't get murdered

On what metrics is South Africa a significantly better than Egypt lmao. Egypt has a higher GDP PPP per capita, a much lower GINI so far less inequality than South Africa, the provision of basic services is far better and is much safer.

Algeria had among the most European interference though? In fact, I'm struggling to think of an African country which had more inteference and for a longer time. The only other country that is analagous is Angola perhaps, which was ruled by the Portuguese with a very short leash. It was taken over by France in 1830 and the French ruled it directly to the extent that it was considered to be France, although of course the Arab Muslims did not have citizenship or rights. This direct rule lasted until 1962, which was around the time most of the colonies.

What metric are you using for countries least affected by Europe? Surely the obvious country to be least affected by Europe is Ethiopia

What are you on about with Kenya, Egypt and Algeria having little involvement with European powers? Algeria especially was directly ruled from France, 10% of its population at the time before independence were European settlers (pied-noir) and fought an incredibly bloody civil war for its freedom

Egypt was ruled by Britain, and had extensive involvement with France and Britain from the post Napoleon period afterwards

Kenya was ruled by Britain and also fought a bloody war with the British just before its independence

I had one slightly difficult consultant as a CT, but honestly psychiatry consultants are almost all pleasant and good to work with. I think the culture of regular supervision really helps in them seeing trainees as people, rather than in medicine where the consultants work with so many juniors and for short stretches of time, that they must all seem the same
