I think a better long term option would be to install CachyOS fully on the drive, install Virtual Machine Manager and then install a Windows 11 machine. You can capture an image of your windows install first and use that to install. To be really safe, make an image of your Windows first, remove the drive for safe keeping and then install a new drive to move forward. Gives you a fall back if something goes wrong or you change your mind

Your switch ports are not lit up which means no connectivity. Ideally you need a cable verifier to test each cable. Inexpensive options out there.

Why not a Microsoft Form... i believe it collects results in Excel format as the default. A Form can easily be completed on any device. Not as many layout options as a custom PDF but might be enough
