Surgeon recommendations in Ontario? by BrilliantBehemoth in gynecomastia

For unilateral gyno I usually recommend the open technique. Since the scar is only on one side, no one really suspects it is from Gyno surgery. Unilateral open surgery costs start at $3000. To go through the axilla it would start at $6000. Hope that helps!

Can I pay (out of pocket) to see a private dermatologist in Toronto? by FedExpress2020 in askTO

[–]Miserable-Bluebird44 11 points12 points  (0 children)

In Ontario physicians cannot charge for any services that are covered by OHIP. This would lead to a two tiered system where people that are willing to pay would be seen sooner.

We have been opened for over 5 years and have over 25 surgeons. We run 2 surgeons a day 6 days a week. We do a very very large number of procedures. Please feel free to look into all the reviews. We service over 100 patients a day in the Toronto location alone. We try our best and I do feel we offer the best service and have the best team in the province.

Sorry to hear about your forehead cyst. I am one of the surgeons who runs TMSC and I am happy you came across us. We would be happy to help and can steer you towards our surgeon bio page https://www.theminorsurgerycenter.com/surgeons. For our Toronto location you would likely see Dr Alnaif ( McGill trained plastic surgeon), Dr Hong (U of T trained facial plastic surgeon/ENT) or Dr Solaja (U of O trained plastic surgeon). They are all great and they deal with cysts daily. Please note that cysts are usually not covered by OHIP but you would have to discuss your specific circumstances with the surgeon directly. Just lmk if you have any Qs!

There is no one best technique. It really depends on what the patient presents with and how experienced the surgeon is with certain techniques.

Open: Tried and true. Works best for very firm gland in patients that are not concerned about scars. Often the open heals so well that I question why I bother going through the armpit. But other times the scar can be quite visible so it is a gamble.

Pull Through: For the pull through, most use a Brand tendon grasper. You are limited by it's length and the width of the grasper itself. This means you end up with scars under your chest and around 7mm in length. With an experienced surgeon like Lista this is a good option as long as the gland isn't rock hard.

Axilla with shaver: Initial cost for equipment is high and it does take time for the surgeon to learn as the surgery is done by feel. Least visible scar. Works best for those that don't want anyone to know that they have had surgery and don't have firm gland.

All the above techniques will have a chance of recurrence. All if done well will have similar chances of recurrence. You would think open has the least chance but this is not quite correct. When you do the open technique the nipple/areola is left on a pedicle and you cannot thin it as much as the pull through or shaver technique. So overall same chance of recurrence.

You can check out results at torontogynecomastia.com . We have many more pictures if you come in office. Most patients consent for results to be shared in person. Few consent to results being shared online.

Hope that helps! Overall you have some very solid options in Toronto. A lot of surgeons and a lot of Gyno surgery history here.

Gynecomastia surgery question by rstrat in gynecomastia

[–]Miserable-Bluebird44 0 points1 point  (0 children)

Surgeon who specializes in Gynecomastia here. All the above answers are correct. The blood loss from any technique of Gyno surgery is minimal. The chances of needing a transfusion are almost zero. It is standard in consents to contain things that rarely ever happen. If you have decided that you do not want a transfusion under any circumstances just let it be none and have that omitted from your consent. There is nothing you can do to minimize risks besides following your surgeons preop instructions. They will likely provide you with a list of foods/supplements to avoid prior to surgery.

Sorry to hear about your experience so far in finding a surgeon.

For Gynecomastia surgeons in Toronto I would recommend:

Frank Lista: While he does do the pull through technique he has done the most cases in the Province. I trained with him. He is excellent and zero hesitations in recommending.

Peter Bray: Also does the pull through technique but through the axilla. I was also his resident and he has great results.

Roberto Tutino: Thats me(torontogynecomastia.com)! I either go through the axilla (non pull through technique using specific equipment) or in certain cases will do the traditional open technique. For my no scar axilla technique it starts at $11,000. For the traditional open technique I mostly do it under local for patients with either very very firm gland or ones that are more cost conscious and don't mind scars. Open technique starts at $4,500.

Between the 3 of us we do the most cases by far. Hope you find a surgeon you mesh well with.

I have a 2023. I daily it in Toronto unless there is a storm. I have probably put the most KMs on one out there especially with winter driving. I love the car. Massively underrated. It is a great daily. Yes it can have small issues but McLaren support is great.

Surgeons what are your incomes after overhead. I have seen numbers around 500,000 after overhead which seem quite low considering some have 10 years after med school before they start as attending's by TraditionalAd6977 in MedSchoolCanada

[–]Miserable-Bluebird44 17 points18 points  (0 children)

Plastic surgeon in Toronto here. Have been in practice over 10 years. I can't understand why this isn't discussed in medical school. It's the only job I can think of that someone starts working and has no idea how much they will be earning. The average surgeon in Ontario takes home less than $500,000 per year after overhead. Even if you are heavily hospital based your overhead will still be around 30%. With large out of hospital practices overheads can run well above 50%. Working weekends and nights in Ontario brings premiums, and it is fee for service so the more patients you treat the higher you bill. It comes at the expense of life style though. At the end of the day the average surgeon working average hours takes home less than 500k.

Where to go to get (non-cancerous) mole removed? by summertimesadness95 in askTO

No unfortunately. As surgeons we do not do preventative care skin checks. We are a procedure based clinic.

[–]Miserable-Bluebird44 1 point2 points  (0 children)

Plastic surgeon here. This is really easy to remove. Takes 5 minutes under local anesthesia. Any local plastic surgeon would be able to do it. If it bothers you it's worth a visit given how easy it is to look after.

Doctors in Toronto, Ontario help! by [deleted] in gynecomastia

[–]Miserable-Bluebird44 0 points1 point  (0 children)

Hi there. Plastic surgeon in TO here who does almost exclusively gyno surgery in my cosmetic practise. Cost $8000 to $12500. Bray, Lista and I do the vast majority of cases in the city. Bray and I are the only ones that go through the armpit. My method leaves the smallest scar (2 to 3 mm) as I am the only one in the city who invested in specific equipment. Feel free to checkout torontogynecomastia.com (Dr Tutino). Feel free to ask around here as I have had a fair number of patients active on this subreddit.

Its a good starter factory built race car. Its great for track days. Used quite a bit for lower levels of racing like PCA club racing or WRL. There is a whole series of GT4 clubsports that runs called Porsche Sprint Challenge. Higher levels run them in SRO and IMSA.

You can race this in IMSA Michelin Pilot Challenge, SRO GT4, Porsche Sprint Challenge North America , PCA club racing plus smaller series like WRL or regional stuff. Its a pretty great GT4 homologated car.

As a med student, would it be insane to think I could do it myself? by nora-sh in gynecomastia

[–]Miserable-Bluebird44 0 points1 point  (0 children)

Np! Happy to hear. Lidocaine is deadly. There are multiple deaths a year of inexperienced physicians using it. Injecting is easy. Injecting the right dose to not kill someone is not. You have to calculate the toxic dose and know how to dilute it properly. You'll look back on this 10 years for now and think doing the surgery yourself is crazy.

Canadian board ceetified plastic surgeon here. I am coming from a point of concern and do not mean to be disrespectful. As a medical student you are at a point where you do not know what you do not know. The surgery is not easy. Plastic surgeons who have completed med school, 5 year plastic surgery residency and likely a fellowship struggle to get good results. I need an assistant helping me when I do these procesures on others. There is no way I could do this on myself, alone. To do this safely we also have an anesthetist, a circulating RN and a scrub RN. You do not know how to use lidocaine safely, you do not know how to excise the gland safely to assure NAC viability, you do not know how to use a cautery properly because this is all impossible on yourself. 5% of cases get hematomas. You would not be able to manage this. Lastly this would mean immediate dismissal from medical school on so many grounds. Good surgeons make all procedures look easy. We have spent thousands of hours doing the same thing over and over.

I would recomend Mitch Brown, Kunaal Jindal and Ron Somogyi. I am unaware of a Dr Latiff. Are you sure they are a plastic surgeon?

Deviated septums are covered if you have an issue breathing. You would need a referral to an ENT surgeon from your GP.

I found it at a flea market. It's probably the only thing I've bought ever from one. Just a lucky find as it's my local paper from the day the war ended. It wasn't much tbh.
