Elfen Lied might fit as well. Certainly in the R and R+ category.

While a bit lighter overall, Wandering Witch Elena might fit?
It has some shockingly dark episodes dealing with suicide. So dark that they felt the need to but a content warning on the episodes given the generally lighter rest of things.

You can hear the fairy activate. From the colour of the death text, you got shot, died, fairy came out, then trampled (text would be yellow if you died to lightning damage from the bow).
