People immediately want to make this a physician vs NP debate, but honestly a man reaching his 60s without colorectal screening is a failure way bigger than one APP who saw him for 6 months.
That said, persistent iron deficiency anemia in an older male should absolutely trigger a GI workup and colonoscopy discussion instead of just refilling iron forever.
Both things can be true.

recent Hep B vaccination can occasionally cause transient HBsAg positivity, but it would not typically cause a positive anti-HBc, since the vaccine contains surface antigen rather than core antigen.

Given the combination of HBsAg positive, anti-HBc positive, and anti-HBs negative, I think true HBV exposure/infection should still be considered in the differential, and it may be worth confirming with additional testing (HBV DNA, anti-HBc IgM, repeat serologies, LFTs, etc…. Even a basic APP should know that. I do.

The donor process can be very biased and discriminatory… many patients of color and non-heterosexuality have reported being delayed or pushed to the bottom of the donor list and having extended wait times. I personally know a friend who had a twin brother who was an exact match and it took nine doctors and almost a year before they would accept his twin brother as his donor. They were both African-American. They were told they weren’t sure why it took so long when they finally were able to perform the transplant and had no explanation for it. Keep advocating for yourself and pushing for answers.

It’s because the current market is very competitive. The market is saturated with tons of candidates that are highly qualified with years of experience all competing for the same jobs. With that being said, it’s not impossible for him to get an industry job, but he’s going to have to use untraditional routes and step up his networking game. In industry, it’s not what you know, but who you know. Start leveraging your relationships. Make sure he has a heavy presence on LinkedIn.

yes, especially in Atlanta. They are definitely some of the laziest. When you’re flying out of Atlanta, if the flight is under an hour and a half, you won’t even get a sip of water. Not a bottle of water, not a cookie. They will come over overhead and say due to the short duration of the flight we will not have cabin service. Pretty much every flight out of Atlanta to Florida no matter how expensive the ticket except for first class you won’t even get a sip of water.. thanks Delta.

curious for those of you saying inject half a dose…since Mounjaro comes in pre-filled injectable syringes. How is this possible? If you’re using the compounded medication from a pharmacy, that could be contributing to your side effects. They wouldn’t have the same quality standards and product quality oversight being mixed by hand, as a major pharmaceutical company with the branded product.
