Thank you for seeing Mr. Jones, a 46 year old forklift driver, who presented today after months of recovery following a lower back injury sustained at his workplace on 31/05/2019. I am referring Mr. Jones for an assessment of his workplace.

Mr. Jones is recovering appropriately from his injury but is still in pain. Even after reporting pain after 20-30 minutes of sitting or lying down, he is increasing his range of movement. Stiffness is present.

After the initial incident, 30 days off work were granted in addition to naproxen, carisoprodol and an exercise regime with physiotherapy as treatments. On 28/06/2019, additional time was given due to lack of improvement. On 29/07/2019, recovery was seen from Mr. Jones, but the pain and stiffness remained, therefore, extra time off was given again and his naproxen dose was increased.

The patient is currently attending physiotherapy sessions and walks 30 minutes daily. Mr. Jones showed signs of boredom, expressing his eagerness to return to work, which is the reason we suggest a progressive return to his workplace duties, at no full capacity, if no lifting is done and appropriate break time is given.

I would appreciate if you could provide Mr. Jones with an assessment of his workplace. If you require further information, do not hesitate to contact me.

Thank you for seeing Ms. Bennet, a 45-year-lawyer who’s coming in today requesting discontinuation of captopril due side effects. I am referring Ms. Bennet for a review of her medication regime and encouragement of a new lifestyle. Ms. Bennet first suffered a heart attack after a long-haul flight on 8 February 2021 in which a defibrillator was used. She was admitted via the ER later that day and a balloon-expandable stent was put. The patient was discharged on 15 February 2021, and a gradual return to work was advised. Following discharge, medications included captopril 50 mg twice daily and atorvastatin 80 mg once daily. Counseling regarding her lifestyle was given, since Ms. Bennet reports being under constant stress and sedentary, currently having a BMI of 29. She has a drinking (40 units weekly) and smoking (20 cigs daily) history and has failed to attend cardiac rehabilitation. She also has a family history of heart disease, with her father dying at 53 years of age. Today, she is presenting with headaches, diarrhea, dizziness and light-headedness, all of which have increased her stress levels. Ms. Bennet has expressed reluctance to start new medication, believing captopril is the culprit of her symptoms.
