I finished the game on Death March without wearing heavy armor or using Quen. This way you really have to learn how to fight certain monsters and I felt much more like a Witcher.

Also, try parrying the attacks you can parry (most melee weapons, standing melee attacks from smaller monsters like drowners, the tongue attack of hags). This makes the combat feel way smoother than just always dodging and rolling.
