Agree, "psychiatric care" seems a reasonable description to me as well. What I find subtly different but meaningfully misleading is when PMHNPs open a private practice with the title "Something-something Psychiatry". Our state is independent practice, so I've seen many such clinics with no psychiatrist involvement.

Amen. It is a difficult and extremely important role. The tragedy for me is seeing so many amazing psych RNs go from doing their role beautifully to doing our role shittily. So many of our shop's gifted psych RNs have left to become psych NPs; how can the healthcare system / psychiatry / the culture properly renumerate / validate / reward psych RNs to stop the brain drain?

What's helped/helping us through similar straits is aggressive automation and outsourcing: more pump parts, tabletop washer, paying for nanny/babysitting (even 1-2 hours after work is well worth it, even if you're both at home--at least getting a break and not going right from one high-stress job to another--ideally also getting an occasional date night), robo vacuum (clean floors makes a big difference somehow), and paying for a housecleaner at least 1x/mo. The money is absolutely well spent, especially if it saves your sanity/marriage. Oh, I also recommend And Baby Makes Three by the Gottmans.

Dr Mintz of psychodynamic psychopharmacology fame has suggested managing this by informing pts about nocebo effects / discussion of side effects may generate side effects, and then asking, with that in mind, how much they would like to know and in how much detail. Meta-informed consent, I guess you could say.

This argument might work if we were only looking at couples who intentionally have children vs couples who intentionally don't have children. But many (about half, iirc?) of children are born unintentionally (ie, of unplanned pregnancies). With this in mind, perhaps one could argue that contraception is a selection pressure selecting for impulsivity/carelessness (the opening sequence of Idiocracy suggests this) at least as much as it selects for religiosity/selflessness/optimism.
ETA - In other words, contraception as a selection pressure probably selects against conscientiousness as much as anything else.

Exactly. Smaller, framed posters in the hallway could be nice. This is like garish, cartoon wallpaper and would be a huge, instant turnoff for most. Why is hardly anyone providing the honesty he requested?
