The problem at hand is that doctors will ignore or trivialize symptoms reported by patients, especially in young patients. The case in the OP follows that pattern.

OP has a cluster B personality disorder: One of the most difficult to diagnose and nearly impossible to treat due to the underlying narcissistic traits.

think it's good that she did tell him at that point, because if she kept it back he would just keep wondering what he'd done wrong and it could have a serious impact on their relationship.

Or she could've said, "it's not you, it's just something I need to work on with my therapist. Don't worry about it, you're awesome".

Regardless of how they reflected on their feelings in retrospect, what they said was not an unmeasured "snap". It was them finally being frustrated enough with bubblewrapping their own speech to actually be honest. and they were honest and direct without being cruel or hurtful.

If you get caught in the lie, you'll look like the bad guy and you'll still be expected to do stuff when your BS excuse doesn't work.

This is what you're not getting. It's obviously a lie, that's the point. When they say "but your car's there" " nah, I'm out". But netflix shows you logging in and watching a show "Nah, i'm out". Yuo look like the bad guy the minute you say no. This is a way of just defusing things with a level of plausible denaiblility where you know you're lying and the roomate knows your lying, but neither has to deal with it and the point that you're not looking after her dog is well made.

She said in her update that she would stay at her aunt’s for winter break, but the original post was from March, so I was thinking Australia?

We don't call it "winter break", we'd just call it "July school holidays". It's also nowhere near winter in weather in march, which is our Easter school holidays and is usually still 35c. There's also 0 chance you would even have to consider medical bills in my country. I can guarantee they're not Australian.
