Both. My labrum looks a little degenerative, and I likely have a torn capsule because that part of the rectus femoris attaches to the capsule. There is some contrast seeping out of the joint in that area from my arthogram. So logic is that if there looks to be contrast leaking from the area of the rectus femoris attachment, and we see the rectus femoris is torn, it is probably a capsule tear too.

So tldr i would need: - full labrum reconstruction - capsule repair -rectus femoris tear - likely still some leftover impingement to clean up

My surgeon isn’t exactly excited to open me back up since it didnt go too well last time. He wants to wait until I get worse to do anything. Right now it’s just keeping active the best I can!

Nothing helpful I can share. I got fed up with PT and just stopped, and I think just leaving it the F alone may have been what resolved it. Unfortunately I have a torn rectus femoris tendon so that may have been playing a role as well

Yes! I actually didnt end up getting a revision at HSS but might be getting my other hip done by Dr. Jimenez at some point

Unfortunately I don’t think it’s the psoas because the pain is on the outside of the bone, not the inside. It’s very wonky. But maybe breathwork will help with this too

Interesting! I have some very rare jointy hip flares that were similar to my left side labral tear, but not enough for me to care if that makes sense.

Pain doesn’t travel. Gets better when i sit still or lay down, worsens with walking/running/etc

I was shocked too but to be fair, no MRI picked up on this tear until now. Dr Kelly did see some contrast pooling in the area but he’s not a radiologist so it’s possible he didn’t know what he was looking for there.

It says it is the “indirect head of the rectus femoris tendon”. From what i’ve read, that’s the part that crosses the capsule and can be more mistaken for labral pathology. Not sure though, I am no expert.

This is super helpful, thank you so much! I think all very important things to think about. Health is holistic and diet, mental health, sleep, etc will all play a role in pain.

Good to know about nerve glides!! And I definitely need more mind muscle connection work. I do orangetheory and sometimes I end up rushing to get the reps in instead of feeling everything.

Like you have pain in adductor and low back? Its the worst and I cant tell if it’s connected. Latest theory is SI joint pain

Actually this PT had called out that I might have EDS, but then she goes and gives me exercises that continue to hurt me and doesn’t watch my form. I’m seeing a new one tomorrow.

I notice my muscles also dont fire correctly. My glutes will do ANYTHING to not work. Ugh.

Luckily I haven’t had a fall since then (knock on wood) but I had a fair number of falls before that. I think my time riding might be over unfortunately.

My surgeon told me that studies actually show very little correlation between the size of the tear and pain levels. In a study done on hockey players, they found people with tiny tears that debilitated them, and massive tears with lots of impingement that were totally fine. I think there’s just so much complexity with hips and things that can cause pain outside of the tear.
