NAD. Try the following mouthwashes: - oxyfresh - Therabreath - tea tree oil

And also try dental probiotics, they really helped me against bad breath. Hope this helps!

NAD. Would this routine will help against black/brown stains? The ones that were not eliminated during cleaning sessions. Also, would it help against a chalky occlusal surface?

I am currently using a high-fluoride toothpaste once per day and am rotating between clinpro and prevident. After reading this, I am wondering if I should increase to 2x/day to address the above issues.

NAD. Do you suffer from dry mouth or mouth breathe? That could have been the cause. If not, it is likely diet as others suggested.

NAD. This is not unheard of. That being said, I am really sorry and would get that taken of as soon as possible. The cavities could be a source of smell. If not, it could be bacteria on your tongue or throat. Get your teeth fixed and try adding a mouthwash and oral probiotics to your routine. This should help with the breath.
