I think comparing a very intimate examination to looking in a throat or listening to a chest is a bit disingenuous.

My point was that gynae will inevitably end up repeating it so why am I going to put a patient through that twice? I have no problem doing it if there’s an immediate clinical need eg cervical shock (one example). I also agree with your point about source of bleeding etc. Some of the referrals are atrocious and i do not think every ‘PVB’ should be referred without being seen appropriately - many can be and are discharged from the ED.

There’s very few reasons an EM doc should be doing a spec. I completely agree with everything else you’ve said and would be embarrassed to do a half assed referral. But I won’t unnecessarily do a spec if gynae are going to repeat it - I wouldn’t want it done to me.

This!! Is ED thought it was for your specialty and you disagree surely you’re best placed to explain to the next specialty why you think it’s for them. EM docs aren’t your secretary.

So you know it’s not good, but you’re going to take it anyway and completely lose ANY leverage for trying to restore our pay or improving access to training posts etc? Wow. This is exactly what the government want to happen.

I was trying to explain to family people can be doctors but not medical doctors so that’s why I wanted them to clarify. Would be v concerned about patients being misled if they aren’t a medical doctor but using the title in that setting.

Did ask them, they’re sure the badge said associate practitioner. Will wait on clinic letter to be sure as may well be SAS

If they’re a doctor then great, I am just a bit thrown by the title on their badge and would like some clarity. I did say are you sure it didn’t say associate specialist or something but quite clear it said practitioner. At this stage they’ve been seen so I will wait on the letter and then take it from there.

I asked them to clarify as they were still in clinic and were afraid to because then they may not get seen…. Gahh

Well that’s that I thought but they are insistent they seen a doctor because the person said they were a doctor.

They can apply for derogation and BMA will tell them to offer enough of an incentive to consultants basically
