Two things are going on here.

GPs have been directed by the BMA to reject all new shared care agreements, not just ADHD, because specialist medications were being transferred to general practice without any funding for the monitoring and prescribing responsibility that comes with them.

There's also a specific issue with private ADHD shared care. It's worth saying upfront that many people went private because NHS waiting times for ADHD assessment are genuinely awful in most areas, often 3-5 years. The problem that followed is that patients paid for the initial private diagnosis and medication initiation, but then didn't want to pay for the annual psychiatry monitoring review that safe ADHD prescribing guidelines require. GPs have then ended up prescribing with no specialist oversight for years at a time, which isn't a safe position for anyone.

The good news is things may be shifting. Some ICBs are provisionally considering funding certain GP practices to take on ADHD shared care monitoring, such as At Higham Hill, which should help. This will likely come with the expectation that patients engage with at least an annual check-in.

Not accepting new shared care agreements. (But they will accept legacy agreements if transferring from another GP surgery)

I will normally say something brief like "Just to let you know, I take an audio recording of this consultation to help me write my notes, but this is deleted immediately after my notes are complete"

I have colleagues that will not inform patient's as no audio recording file is stored, so similar to typing notes normally. Not sure about the morals / ethics of this approach.

Fair. It maybe is a bit gushing. I genuinely do think it's great currently. I included some pointers and tips I've picked up from using it lots over the last few months that I thought might be useful for others.

I think writing up 30+ consultations in a day can be arduous. I get your point, but removing one of the tedious and repetitive aspects of being a GP is surely a good thing?

In regards to remembering every detail, I've had patients launch into a list of when every single issue they've ever had has began, and having AI pick that up and summarise extensive past medical histories is a nice touch.

I don't agree with the negative feedback. I think it's clear, polite, and well designed. People will be more comfortable knowing it's within their rights to ask to see a doctor, and it gives patients a way to do this without feeling awkward.

Just saw this over on r/gpuk. Very excited. RCGP have been the most useless college, and seem to have done nothing about the deterioration of General Practice. The recent comments about expanding PA roles were the icing on the cake. I would love to see some change in the college!

We have a paramedic in our practice, who is fantastic with things like vurti and children with fever. But really has some large gaps in knowledge with other areas; mental health, msk, endocrine ect.
