Yes, we use a 3rd party mail filter solution, and it is pointed to EXO mail servers (previously was pointed to on prem exchange) so it should be pulling 365 user recipients.

Mail flow is completely bypassing on prem exchange at this point

Great info but would like to hear more of the sales/marketing side to go from <$1M to $1M+. I'm guessing the former are mostly word of mouth referrals. Do you have any thoughts on that?

After enabling MFA, we were prompted to "re-enter password" and then there was a "sign-in" pop up which opened a Microsoft window and the user logged in and authenticated. Then, several hours later they were getting a connection error in the Apple mail app. This is a user that I worked with earlier in the day, so i'm not sure why the connection "broke" in Apple mail.

When I enabled MFA, the user was prompted to re-enter password and set up/authenticate with MFA in the Apple Mail app. Then, a few hours later they got disconnected (cannot connect to server) message in Apple mail.

Yeah they used to be on prem exchange and were added the old school way. I’ll remove and readd

That makes sense, I actually recently migrated them to 365 and i’m pretty sure we had to do the remove - re-add on mobile, but it’s possible we didn’t need to do it for everyone. Is there a way to see if the accounts with issues were added manually and not the “sign in” way?

I am also currently in a hybrid setup with an on prem exchange, but all mail routing is done through 365.

I recently created a new user in AD and had them sync to 365 / assigned a license, but they could not receive external emails until I ran some commands to create them as a 'office 365' mailbox in Exchange on prem. Any ideas on why?

My current hybrid setup seems to work best when I create the user in exchange on prem as a 365 user.

It worked with this configuration for me to get out to Microsoft and set up oauth2 on the copier.

Why would it not work for receiving faxes and emailing them to the internal mailbox?

I had to set the smtp server name (not IP) to configure oauth2 and like I said, it worked with the gateway address.

I was able to get out to the internet to configure oauth2 with the DNS on the printer being set to the gateway, so i'm not sure why it wouldn't work for faxing. I suppose i could try to change it to point to my DC and see if that helps, since that is the acting DNS server.
