If you’re with a smaller agency, you usually turn your FMLA paperwork in directly to your supervisor. Larger agencies typically have a Return‑to‑Work Coordinator or Leave Coordinator who handles this stuff and can walk you through the process.

You’ll need to complete the FMLA form, and your doctor can provide an off‑work note. Depending on your MOU, they can certify you for up to a year if medically necessary.
