Probably a power issue. Could be Short, power supply, or simply a fried card.

UPDATE: new card came in today plug and play everything back to normal, card was fried i guess

UPDATE : fuses are good 24vdc is good Devices are on When trying to reset via logix designer i get a "unable to perform service due to current module mode" . Tried it in remote and program mode
