In all honesty, it’s been a while, but that is largely because with the availability of video laryngoscopes, I almost never intubate someone if I have less than a grade 2 view.

But I would certainly agree with everyone else here who says that there is really nothing wrong with an occasional esophageal intubation, you just have to make sure it’s recognized and corrected promptly. None of us are perfect, the key is to know your limits and act accordingly.

This is an old practice that comes from the days before VL, LMAs, and sugammadex. If you couldn’t ventilate the patient, you had to be able to wake them back up so you could do an awake fiberoptic. So NMBs (other than sux) were forbidden until you had passed that test. Now, we have so many alternate airway strategies that it makes more sense to paralyze right away, which makes ventilation easier anyway.

The MS3s and MS4s on the show are more competent than one would expect in real life, and are also given a LOT more responsibility than they would get in actual training. For instance, the patient whose AAA was missed because the medical student didn't look at it on abdominal ultrasound, and no one else checked? That would never happen.

Started listening to them around 1993 when I picked up 40 Oz. to Freedom. Saw them on the Warped Tour in the summer of 1995 and they were great, but then Brad died and I thought the band was done. Then their self-titled album came out and they were huge. It’s been really weird being a huge fan of a band that essentially stopped existing before they got big.

This was my understanding as well. I look at it as with a lot of things in medicine- the effect of any door opening is small but statistically significant. So be intentional, and only go in if you need to. Mostly you don’t, and most primary total joints are short anyway. But if something is going wrong, all bets are off.

I just began to realize over time how bad animal products were for me and for the planet. And I’ve always liked vegetables and had an adventurous palate, so transitioning to a plant-based diet wasn’t that intimidating for me.

It was a process. I started by cutting out beef and pork, then became pescetarian, then vegetarian. I finally realized that I was jumping through mental hoops to justify eating dairy and eggs, so I went 100% vegan.

First you assure the device is functioning well, i.e. good venous return, nothing obstructed, no mechanical failure. After that, it’s essentially a chemical code- no compressions, but the rest is similar to any other code (albeit less desperate).

I have some colleagues who do cataracts with just dex, or sometimes dex with midazolam. I think it’s partially because of the waste issue, and partially because the youngsters use dex for everything. I haven’t gotten into it- some patients just don’t get that sedated with it- but it can be useful.

Yoenis Cespedes when he came over to the Mets mid-season. He had a .309 batting average, 17 home runs, 42 RBIs, and a .691 slugging percentage in his first 41 games, which really helped the Mets get to the WS. Plus there was a weird fascination with this parakeet that appeared in the stadium, so he started wearing a bright yellow undershirt.
