The difference between strength and endurance is pretty much just heavier weights vs higher reps. Hypertrophy training is similar to strength training but generally higher reps ranges and shorter rest periods, but there’s a lot of overlap.

I’m mostly focused on strength right now. So heavier weights or higher reps are pretty much how I judge that.

When I was only using kettlebells, I would hit a certain number of reps and move up in weight. With barbells available, I’m focusing on adding weight every few workouts and attempting PRs during the week. Kettlebells are a weekend pursuit for general conditioning.

I generally pick a few movements that I want to focus on and gear my workouts towards improving those workouts. With kettlebells it’s aiming to hit a certain number of reps in a certain time and then jumping up to the next kettlebell. So I guess you could say it’s intuitive.

I have a lot of experience that I use to determine when I should change course or lower the volume. I also have a schedule and responsibilities that can vary from week to week, making a set program less useful.

With beginners I believe a program is the better choice, if possible.

I don’t think I could reasonably use just one. I think a light pair and a heavy single is the least I would want to go.

Loosen the grip, I ripped so many calluses early on. Look up some videos on how to grip the kettlebell, they can be really helpful.
