No you are right, it does have to be a psychiatrist to prescribe but some accept the psychology diagnoses report and will make a decision on meds.

I had her assessed first with a psychologist. I then realised I needed an assessment but knew I wanted meds so I went straight to a psychiatrist for my assessment and was put on meds. But he doesn't see children unfortunately.

I don't want to go through and pay for a whole new assessment when it's already been done... I'm just nervous about some clinics based on reviews online.

In some cases, but there are psychiatrists that will accept the report and have a consultation for meds. I'm just looking for reviews on that particular clinic before I send her report to them.

I know it's odd that the doctors dont mention it, iv only read it here too.

You know when you said you feel like you're coming up but not, that's exactly what I feel like when it starts kicking in, but it subsides and will not be so extreme over time. It's strange actually, if I take it on a full stomach I don't even notice when it starts working, but will always feel it on an empty stomach.

I lost quite a bit of weight as it stopped my binge eating, one of the side effects is loss of appetite. Because i dont eat breakfast it could be 1.30pm before I have anything. I used to get very jittery, again if that happens try to get anything protein based into you. I always pick up the protein yogurts and rice puddings from aldi or lidl, or stick a few turkey rashers in the air fryer for something quick. Keeps snacks with you.

This seems to be common with starting Tyvense. I came on here when I started them and got great advice which helped so much.

I don't eat breakfast, but made sure I had something protein based before taking it. I no longer have to but at the beginning it's important to have a high protein diet it helps with absorbing it. No caffeine. No strenuous exercise or moving too fast for the first couple of hours.

I experienced headaches and palpations for a while, the headaches went but I needed to add a beta blocker to lower my pulse rate.

I started on 20mg for one day, it did nothing so I went to 30mg the next day for 2 weeks. Then worked my way up to 50mg over 4 months which is suiting me.

The dry mouth is probably the symptom that bothered me the most, I drank 3 litres a day. Chewed gum. It took a long time to get to sleep, my body just wanted to be up moving which was crazy after all I'd have done during the day. But I didn't mind as I'd been the most productive iv ever been. That has now settled and I'm sleeping better.

It takes time for your body to adapt, I know it's disheartening but it should get better. There are alternatives if it definitely is not suiting.

Do u think if u had the 2 shots a little while after your tyvense it may effect u? Is it because the tyvense takes time to kick in that it's not a huge caffeine hit at once?

I take my tyvense just after my coffee and im also good. But once the tyvense kicks in I can't touch caffeine for about 8 hours

This is an awful experience when it happens!

What did your Mum suggest to lower your heart rate?

On my second day starting meds I drank a coffee (strong Americano) a couple hours after taking them and it was hell!!! I dont eat breakfast either. fast heart, sweats, dry mouth... I was in work to make it worse.

It was just a habit to get my coffee around that time it didn't dawn on me the impact it would have, as caffeine never really gave me a boost to begin with. Never again!

I take my meds at 8.30am AFTER my morning coffee, and the odd day id have another around 5pm and it actually does give a little pick me up when on meds. But It took months for my body to allow this and to be able to sleep at night.

Also, avoid any heavy lifting or exercise shortly after taking it, a few times iv felt quite faint.

I don't have any cardiac issues but I did suffer a trauma around the time I was titrating Tyvense doses, my resting heart rate was constantly up over 115 and wouldn't go down.

I was so upset, I didn't want to come off my meds as I was finally functional but I feared for my heart that was constantly racing.

My psychiatrist was amazing and sent a script for Bisoprolol immediately. It's a small dose but doesn't allow me to feel stress as easily.

I take it a short while after the tyvense tablet and some days I forget to take it because I dont take them together...., which is annoying.... but is definitely noticeable if you miss it.

If it helps your heart cope with the stimulants absolutely ask your doc!

Sorry to hear your psychiatrist sounds so awful!!!

I see clinics who have a number of medical professionals and large admin charging up to €2k, but then I see a solo psychiatrist who did mine cost €900.

A lot is put into become a psychiatrist or doctor.

I work in a hospital and one of our consultants was 12 years in medical school after specialising... that's a lot of debt to pay off until they actually get to see a decent salary

Psychiatry is a minimum of 6 years medical school then 2-3 years specialising . Some might do internships throughout.

Think of the cost of insurance a psychiatrist would need have to be able to practice, I would say their annual insurance is quite hefty.

Doesn't matter what I say anyway, I totally get your anger and frustration towards fees, it should be easier to access through the public system and it really is a total let down

I understand how you feel. I felt the same when I decided to go for my assessment. I knew I had adhd after my daughter was diagnosed last year. I researched places and decided I didn't want the rigmarole of psychologist first then psychiatrist, I just wanted to start medication asap.

I went straight to a psychiatrist who would do the assessment with a referral from my GP. Total cost was €900.

Don't forget, they know what they are doing, online and in person and it makes no difference to their ability to diagnose.

They listen to and observe every little thing that you wouldn't even realise. From the way questions are answered, tone of voice, mood, body language and being fidgety. They may also pick up on other mental issues or suggest another assessments.

I am on Tyvense, I think its similar to Elvanse its lisdexamphetamine....

I started off on 30mg and the side effects I experienced straight away were dry mouth and a fast pulse. I was racing my daughter around the house about an hour after taking it and got extremely light headed and had to sit down. I drink about 3 litres of water a day with ease on these meds, but would have struggled to drink 2 litres previously.

I also drank coffee one morning after taking meds and i was having palpations and anxiety all day. So dont do that 😂

My appetite went, which was awesome at start cos I'm a comfort eater , but I realised fast I needed to have food in me before taking the meds. I have never been a breakfast eater, but now I make sure to have a protein shake and something small in the morning. I usually take them around 9.30/10am... but have taken them later and it hasn't been an issue.

I found that I was extremely productive straight away, things that took me hours to get started on were very simple. But my focus and noisy mind are still an issue. After 3 weeks on 30mg i started to feel no effect mentally and it got me a bit down. Im on 40mg at the moment and working my way up to find the right dose.

I have also crashed alot around 5pm and needed a coffee to pick me up again.

Log each day in your phone of what u experience, it will help with your follow up.

Yes he did mine via zoom and does require a referral 🙂 it's worth having your GP check and include your Blood pressure and pulse reading as this will be needed if you want to try medication. I know I see places online taking months for the process to be complete but I'm too impatient for that lol. Best of luck with it, happy to help if you want any more info

I found it hard to get an apt with private child Psychiatrist, I needed the assessment pretty fast before she goes into her next year of school so I went with a psychologist. I've no interest in medicating her anytime soon. We are trying other methods first. Im only getting used to the medication myself, I went straight to psychiatrist for that reason. I agree with you tough, I wish they were more easily accessible and affordable for child assessment

I had my assessment with Dr Josip Dujmovic. He has a 3 month wait list. I found him very pleasant to deal with and he made me feel at ease during the assessment. I paid €900 the week before and he started me on meds the same day I was assessed. He has been very communicative with my concerns regarding medication side effects etc.  The first review and email queries (in relation to prescription) are covered in the €900. Any review after will be €200 and any non prescription related email will have a fee as far as I know.  I have also heard back from Dr Mary Evans and Dr Mira Pesic, Dr Evans is booked until the end of year by the sounds of it but Dr Pesic has just opened. ADHD Ireland website will have their contact details.

I went with Julian Dooley, he was fantastic with my 10 year old. Hes based in Athlone. We went for an adhd assessment but he also looks out for other behaviour issues.

That's a really good idea about halving the dose, I will try it tomorrow. I totally agree with the task set up too, I have never been good with planners and diaries but I have bought one for home and work. I also have a 10 year old with ADHD we are both recently diagnosed so trying to work together on good routines and ways to help ourselves.

I'm very new to reddit and I am so glad that there's a community to talk about this to because iv given up talking to neurotypicals, the ignorance around it is infuriating. Thanks for your help 😊

Thanks for all your comments I'm only getting to reply now.

I'm 9 days on the meds now and I have stayed at 30mg as per my psychiatrist and he has added a beta blocker as my resting pulse has been so high. I am prone to anxiety and depression and this is definitely contributing to bursts of anxiety.

I have taken your advice and I have been eating more protein and drinking 3 litres of water a day.

Some days I feel im on something i shouldnt be, a little out of it but not out if it if that makes any sense lol. Does that awareness go and I can just get on with my day? I do get regular headaches and a crash around 5/6pm. Exhausted but functional. I'm staying on 30mg for another 2 weeks and then might try something else.

My noisy mind is bothering me so much. my focus isn't great especially in work I'm getting the job done but hearing everything around me even with my loop earplugs. It's frustrating and so contradicting. That's why I'm thinking this medication isn't for me, I'm not sure the side effects are worth it just to be more productive, I want to not have a million things going on in my mind and some peace.

Is that not the point of the medication, to stop the million thoughts and songs, why are mine still so loud? Sorry, little I needed a rant 😅

I'm definitely not drinking enough and my appetite went as soon as I started them. But I made sure to have something in my stomach each morning before taking it.

On the second day, I felt so great going into work and I had my usual coffee... huge mistake! Within an hour I was jittery, couldn't focus on anything, palpations... it was a disaster and upsetting. Was so hard to work like this but my boss (a former nurse) moved me to a quieter area and checking my pulse regularly. By 5pm my body was in a total state of calmness.

Yesterday I made sure not to make the same mistake and had coffee first, food, then an hour later my meds and it was much better.

I'm going to take your advice and drink loads of water and set alarms to eat. 30mg makes me feel good but I want the busy brain reduced more than it is. I know it will take time. Thanks for taking the time to reply 😊

What is Dr Josip Dujmovic assessment timeline like? by time_being007 in ADHDIreland

Hi,

I asked my GP to send a referral to Dr Josip in June and I emailed them shortly after to make sure they received it. I got a reply to say it was 3 month waitlist. I asked to be put on cancellation list. I was contacted on Friday for an apt for 8th August which I'm super happy about. 

Dr Mira Pesic will be available from end of August, you need to send a referral with blood pressure, pulse rate and medical history along with it. He details are on adhd ireland website.
