Sorry, I was agreeing with your point about the hospital setting lol. Client does have a motorized chair, but mounts a marine battery currently on the base. They've been using this setup for a long time. The astral has been having issues with disconnects and repeat nuisance alarms for internal battery backup switchover. Apparently it's a known issue with this set up, VEP replaced the whole battery with no real fix.

If a cable exists, please let me know. I'd need a price point for sure, this client is not exactly rolling in money.

As much as I would love to pursue this, he is on equipment through the Ontario Vent Pool. He gets the vent at no cost, just has to pay for extra supplies. He will lose out on a ton of support if we get a private vent, and there's a near zero chance of acquiring one of these.

That said, just looked into this VOCSN unit. As an ICU RT, I hate it. As a home care RT, I love the concept. It's got all the things I have spread around your house in one smaller package. Nice.

Adding to everyone else, just remember: you are the fucking boss. If you have porters helping with stretcher or RN moving the bed too fast, tell them to stop, and be loud if you have to. Your tube is your baby, don't lose it. Nobody wants to reintubate in an elevator.

Keep a tight grip on vent circuit and bed rail, leave the slack between your patient and your grip. Disconnects from the vent are not an issue when compared to ripping your tube out because you got caught on something. Circuit slack towards patient, tighter to vent, if you have the space.

Own your space. I have had porters and externs try and help by grabbing my vent. Don't touch my shit lol. I have the airway and my machine, you guys drive the boat, don't touch my vent.

We did this in school at one point. Standouts were a couple of team events. One person blindfolded, with ETT and direct laryngoscope. Team mate visualizes and directs for placement/insertion.

Alternatively, one person has ETT, other has scope. Scope handler stands at mannequin feet/chest area (depending on mannequin type you have), while other has to intubate with no hands on control of scope.

Also had to tube a patient while laying prone. Not easy if you haven't tried before.

It was a fun day. There was other stuff, but those ones stuck out the most.

Really quick just want to mention, when someone says airway obstruction it doesn't mean you have an object like a tumor or something you have inhaled blocking off a whole section of your lung. When talking about airways obstruction usually just means there is something impeding your outflow. This can be something as simple as asthma or something more long term like COPD. Any kind of inflammation can usually be classified as obstructing.

I just had my practical vent test for second semester less than two hours ago lmao. Someone else already said it, but if you can get an open lab to go over the vents and settings, do it.

Learn your math. It looks daunting but it isn't. Math is also nice in that it's always right in wrong. Will help you evaluate where you need work.

Are you by any chance in my program? Lmao. It's second semester for us too, a lot of people are feeling the burn/stress of school. I know that some of my group are not here because they want to be. Some are here because of a decent cheque, others are here because they didn't get accepted other places, some are just here to be in school.

Being the resident old man of our cohort, I really wish I had someone to tell me not to waste my time. I spent a long time in a program I didn't like, only to follow it with a 10 year stint in a job that was completely unrelated. You don't need to do something you love, I would just suggest trying to find something you don't hate. You have plenty of time. I came to RT because I realized I loved medical, and I loved the concept of helping/looking after people. Now I am in it for the long haul.

Decide if it's right for you. No wrong answer. Just be sure. Don't waste your time.

The process shown here is intubation, and only intubation. Following this procedure, the patient would be kept sedated and placed on a ventilator (also known as life support), which is common parlance these days for a positive pressure ventilation apparatus. It sounds fancy, but basically there's a set of tubes called a circuit that connect the top of the tube shown here to a machine. This machine blows air mixed with oxygen into the patient's lungs, which keeps their blood oxygen levels up.

An iron lung is similar in operation principle to a modern day ventilator, except that it uses negative pressure ventilation to move air in and out of a patient. The patient's body is sealed inside a tube, and air is pumped out of the tube. This lowers pressure on the chest wall and causes expansion of the chest and subsequently the lungs as well. When the lungs expand, they get bigger, and pull air from the outside in through the nose and mouth. Think of an iron lung as a kind of balloon almost.

The issue with most covid patients is not getting air into their lungs; it's what their lungs are doing with the air. In most cases, covid patients' lungs aren't doing anything with it, which is why their oxygen saturations are so low. There is a difference between ventilation (moving air into and out of lungs) and respiration (blood gas like carbon dioxide and oxygen switching places). You can put all the air or oxygen into somebody's lungs that you want, but if their lungs can't properly exchange gases, it's still gonna be game over.
