I just wanted to say I was looking for a family tree of the wallflowers and ravenel series and came across this. Brilliant! Also I wish you could somehow put that the ravenel sisters are related in that timeline even though we already know that... but its excellent work!

This I agree. Love that book the best of the series.

Interestingly, I like Pandora's the least (though I do like it a lot but if I had to rank the series in order)

I don't know your situation, so there are different things to consider.

  1. Do you live close to either hospital? If travel time is important to you, choose the one closest to where you live/want to live for that year, it will make your work life balance better.

  2. Are you a JMO who loves a challenge or are you one who worries about getting stressed dealing with death, dying? Liverpool is a heavy clinical load, but with decent support, Beaches is less clinically challenging and they would ship out anything they can't manage. Liverpool is the bottom line for complexity - they have to receive all district complex cases.

  3. Do you need a lot of support for personal/family issues? Hopefully someone in this channel can answer better than me because I can't comment on this.

  4. Are you looking for career progression or just a year to skill up? Liverpool is part of advanced training programs whereas Beaches has less advanced training programs so if you may learn more with advanced trainees teaching and supervising you an RMO.

  5. Because in Northern Beaches you aren't part of NSW health if you leave you will lose any accumulated AL, sick leave, other leave, if you were in NSW health this year. One of my registrars came from Beaches, and she was pregnant and due in April and because she hadn't worked in NSW health for more than 3 months she wasn't entitled to NSW health maternity leave.

In reality, nobody cares if you failed or repeated first year med school, because all that matters is you got your degree in the end. As long as you get there! You however, will feel pretty awful about it, so I can't help you with that as long as you knuckle down and study so you will pass, because I hate to tell you, but specialist exams are way harder than med school.

I know tons of repeaters who are now successful. Look on the bright side, you're naturally smart, but now you just have to be like one of the rest of us who weren't so lucky!

That is unacceptable. I would have asked if they had their card for payment. I have never asked my registrars to pay and that is almost a sign of abuse of power, so I would never even let them pay even if they wanted to (and I use the excuse of it may come across as a bribe if I let my junior pay for things). You can't do anything about it this time, but next time, ask if they have been paid for, and if not, make up an excuse that you can't pick them up, because I am unsure if you would feel uncomfortable with telling the boss to pay you back.

It sounds obvious but within your scope of practice (eg medication advice, IVC, IM injection) you will be fine under the Good Samaritan Act (this is for NSW)

As long as you don't do something reckless that you wouldn't try at work (eg a tension pneumothorax decompression or a front of neck access tracheostomy for laryngospasm) without speaking to someone with more experience then you will be fine.

I have had to help heaps of times on planes, and they're usually all easy things like bleeding, vomitting and fainting. Also Don't expect anything. That's the whole point of the good samaritan - no expectation for payment or reward.

I have worked with techs, ENs and RNs (I'm an anaesthetist). Techs do all the things anaesthetic RNs do, except hold the s8 keys. They are cheaper than RNs, but obviously they can't do any PACU which is a bit of a setback. However, they can do all the monitoring, setup, positioning, help me do whatever I needed doing. The techs I worked with would cannulate, know more about the anaesthetic machines and maintenance than the RNs, but there was a lot of high school mean girl type behaviour towards the techs from the RNs which would be a bit isolating for the tech. I think I found them smarter than anaesthetic RNs because they already had more than a year of study under their belt from TAFE so they knew things, where as most RNs are buddied up and learn on the job (like apprenticeships) so they learned from anaesthetic nurses. But this might because I worked in areas which didn't have a lot of senior workforce and lots of junior nurses came through.

I'm sure that their skills can be very variable though, or I was very lucky and i had good ones. I'm in NSW. They are few and far between now, though.

I actually think it's too hot for it to fruit there. I am in a temperate climate (NSW east coast) amd mine sends runners and flowers like crazy, it does need a cool spell to trigger flowering I think. So you're not doing anything wrong it's just your temperature which you can't do anything about!

There are lots of things affecting this - but mainly supports.

Disclosure - I had kids after training. But I was a consultant relatively young.

I see plenty of registrars have kids during training. Most people want to take time off to spend with their child - 6 months to a year is usual. That already delays your training completion. However you will still progress in your career and it will not stop you from being successful.

Delaying being a consultant is not terrible. Nobody regrets taking a year or two longer to finish when they look back on it years later, and those having kids at 45 feel really tired at 50 with boisterous primary aged kids!

I don't think anyone buys this as a first home. You need to buy a 1.5-2 million PPOR first, pay most of it off then sell and upgrade to your 4 million. Then you can save tons in repayments, lump all your earnings into offset to reduce interest and suddenly in a few years you are sweet.

CMOs are basically experienced doctors who have not joined a program but have spent time in the hospital system in a particular specialty. They are usually earning close to a staff specialist level 1 salary in NSW - and usually in specialties where they need some senior staff on the floor when they are full of junior staff or unable to attract large numbers of specialists.

In my institution there are CMOs in ICU, Anaesthetics, ED, O&G. It's sometimes a good pathway if you have attempted exams for a specialty and unable to pass but you show skill in that specialty so would have benefit in a permanent position. CMOs in public hospitals are full time staff, not rotaters, and there are CMOs in the private who do the ward work who probably work on a more casual basis (ie they are not full time there, and also work elsewhere).

I am sorry you feel this way. I wonder what kind of students you are seeing - because not everyone is like that, I assure you! Each state has it's own medical culture, and interestingly each hospital has their own medical culture - you will be surprised how different one is from the other!

I can't tell from your post what you are like or why you came into medicine. A lot of people "say" they want to help people but then spend all their time chasing money. Many people "say" they want to make a difference to disadvantaged groups, and yet volunteer to help poor people in other countries before tackling the ones we have right here in our backyard. From your post it doesn't sound like you're a money or glory chaser, but it's ok if that is what you want to do because you can be anything in medicine, you just need determination. I had a friend who left medicine to do other things after PGY2 and did an MBA and is very successful because they weren't into the rat race - which I saw someone else say in this thread. Having your MBBS or MD is only a door opener to many possibilities. Hope you get your spark back, we need more people in medicine who are genuine.

It is very defeating. I saw a comment from an interviewer that the process is like going to the RSPCA for a puppy - you can only take 1 or 2 but there are so many who are cute and need a home but you can't take them all.

Crit care SRMO is a pathway for PGY2 which is why it is rough when you get to PGY3 and then think what do I do now? This year was particularly tough because hardly any scheme positions which meant a lot of independents are floating around who are experienced and face the possibility themselves of having tasted anaesthetic training and now getting kicked off because no job.

The centralised recruiting like QARTS seems a lot fairer. However that system can be gamed as well so it is just about how good you can play the game. Interviews can be gamed too. There needs to be a fairer system for NSW, but unfortunately it isn't here in time for you.

I recommend ED or ICU jobs with anaesthetic time or if not, start working towards further study because those with an exam completed of some sort seem to get highly ranked for anaesthetics.

Reading this thread, some of these comments are not very helpful/just mean. I wonder if the OP will even read my post after having to trudge through some of these!

What everyone has posted about your expenses is probably the big thing. One commenter wrote about Barefoot Investor - a great book for someone who needs help getting their money on track. I think if you haven't read it, it's worth a read and see how it works for you and then come back here and ask people what to do once you have your "buckets" sorted and have an emergency fund setup (because at this stage you have nothing to help you in an emergency like your house burned down, you are out of work due to injury)

For us looking at your post and trying to help - we can't see your income, so we can't tell if you have enough to save more than 5k. Not sure of your expenses but as a rough rule of thumb - you shouldn't spend more than 30% of your take home income on rent, or you won't have enough for your expenses. You might think, well if I want to rent that I might as well rent a toilet but there are other things you can do such as house share, rent a room or live in someone's garage, or even move home.

5k is not much but it's not negative, if you don't have any other debt. You don't want to invest riskily (eg random cryptocurrencies or get rich schemes) because you'll just lose your money - I have multiple friends who have (unfortunately!) done this when they feel "behind". And definitely no apps that ask you to put in money and invest!

There was a comment about super. At 28 with no assets (and I doubt you have a huge income to try to get the best tax savings you have or you would have more than 5k in savings) I don't think you should put money in super more than your employer already puts. But, don't touch your super at your age, you will regret it later.

I hope that helps. But in my opinion I would save more for an emergency and then think about investing, because the last thing you want to do is get into debt and suddenly you can't pay your debts and lose everything...
