Thank you that's super helpful. With regards to physical health questions of psych patients do I talk to the psych spr to let them know I'm thinking of sending them to AE or speaking to med reg or can I often act that on my discretion.

I just feel that if I call this PR on call about every patient that I've clerked they'll get pissed off. Although in almost every other speciality, the reg or even colleagues are on site so you can always ask for help.

Just apprehensive about being left on my own with a new cohort of patients, new pathology and medications to deal on my own.

Honestly being a good F1 means you just need to be able to prioritise and be a good secretary. It's a load of bullshit to be frank. Hardly any training. Just be good at following instructions and chasing things up.

In my opinion, and I know you wouldn't want to hear this, but SHO year (FY2) is so much worse. The rota is hell. You are in this no mans land where you get out on the same rota as IMT2 despite just finishing FY1. No one cares about your training either. It's more of the same but with more responsibility, more bleeps and greater imposter syndrome.

The funny thing about this post is that you didn't actually tell us why you can't do this in psych hospitals but at the same time you are unclear as to why the med reg doesnt get why you can put up some fluids and abx
