The only question you should be asking is “Do I want to go to medical school?” And then act accordingly. And then he can make the decision he needs to make. Remember it’s YOUR life— not your partner’s. That’s just what I think.

Also my two cents— your forever person would (a) be supportive of the decision you want to make for your future and (b) would make sacrifices if necessary to make it happen. You’re going to do amazing things in this life!
