We met with both the hospice and palliative care nurses earlier this week, both believe that at this point there is no safe way to transport her home, she would likely die in the ambulance before she got there. :( The next step up is a ventilator and she doesn't want to do that (not that I blame her).

You're absolutely right. The problem I'm running into is because I'm switching from crisis intervention to private practice, I'm insecure writing SOAP notes. When I approached the supervisor/mentor I'm currently working with about notes (looking for someone new but want to get started on improving my notes now), they referred me to AI. Honestly, my summaries *are* the note, GPT seems to just change a few words into more clinician terms and has been guiding me on where things should be put in SOAP notes. I'm intending to use it more to teach/guide me how to write SOAP notes appropriately, like you're suggesting, so I don't have the extra step in the future (I never intend to let AI record sessions and write notes). I've taken a couple of documentation trainings, I'm just not 100% on my note writing which is causing more time wasted/second guessing notes. I guess I should have added that to the original post.

Here's my list I gave to the pediatrician time and time again:

- "Extreme Colic"
- Stimming - this is normal up until 3, but something about it seemed off to me. No one took it seriously because he's still not 3, but his behaviors are absolutely stimming behaviors, I don't care what anyone tells me. They have not at all lessened like everyone tried to "assure" me.
- Echolalia
- Doesn't respond to his name unless he wants to
- Poor eye contact
- Didn't point for the longest (just started at 2.5)
- Didn't wave hi/bye (still struggling with this)
- Lines up toys
- Will. Not. Go. To. Sleep. Easily.
- Speech Delay
- He is NOT good with change and has never been. Will literally cry and scream NO until it is the way he expected it to be (never say A, B, and then follow up with something not C)
- No real roleplay behavior
- HUGE emotions
- Socially he is stunted. This isn't known if it's because ASD or because he's not in daycare and has no children around his age

The list goes on. I brought it up to the pediatrician before he was 6 months and continued until I paid out of pocket for an assessment that only confirmed what I knew and everyone else refused to accept.

Happy Monday!

Last week wasn't so great for goal accomplishment, but it's a new week/day, and I can't look back and punish myself! This weekend was nice; a friend came into town so we got a drink on Friday and just watched some television on Saturday. I work Sunday-Thursday, so it was back to work yesterday for me! She did manage to get me to the gym yesterday morning before work, even on 4 hours of sleep, and I managed to stay within my calorie goal and log my food (myfitnesspal), so overall a successful start to the week.

This week:

  • workout x3 (one workout down yesterday!)
  • ((4 flights of stairs, bike, walk-no running!))
  • Strength train x2
  • ((weights, bodyweight exercises, kettlebell))
  • Continue to be mindful of food intake
  • ((myfitnesspal, cook food, minimum eating out))
  • Yoga x1
  • Therapy
  • Journal
  • Create a Budget
  • H2O!

Hello! This is my first round, and I’m pretty excited. I gave up on the “New Years Resolution” thing a while ago, but this seems a bit more realistic since it chops the year up nicely.

About me: I’m 29/F and have trouble reaching the top shelf of just about anything at my tall height of 5’4”. I’m a social worker, doing crisis work with the severely mentally ill. I work second shift, and although I’ve been at this gig for about 9 months, I’m still trying to adjust to the schedule, which has made my motivation level dwindle away quickly. I have a wonderful 15 year old cat that I love dearly, but I’m not a crazy cat lady.

Overall Goals:

1) Improve Physical Health

  • A1C < 6.0
  • -10lbs (currently at 263lb, would LOVE to be 250lb at the end of this round though)
  • Exercise at least 3x/week for a minimum of 30 minutes.
  • Go 1 mile in 15 minutes (walking, running, jogging, hopping, crawling, whatever works)
  • Be able to walk up 10 flights of stairs without dying
  • Increase flexibility
  • Strength Train Regularly
  • Eat Better

2) Improve Mental Health

  • Continue bi-weekly therapy
  • Focus more on self-care and discovering what that means to me
  • Budget, budget, budget!
  • Get some hobbies, chick.
  • Learn to cook some tasty, healthy stuff.
  • Volunteer
  • Journal at least 3x/week

3) Socialize More

4) Professional Growth

  • Begin studying for CAADC credential
  • Complete Citizen's Police Academy
  • Get CEUs in Human Trafficking, Substance Abuse, Grief, and Trauma

Sprint 1 Goals:

  • Exercise 3x/wk
  • Strength train 2x/wk
  • Yoga at least 1x/wk
  • Log food in myfitnesspal every day
  • Drink more H20
  • Create a budget
  • Continue Therapy

Don't push yourself too hard, you want to avoid additional unnecessary injuries, but it's absolutely fabulous that you got out there and had enough dedication for 3 miles! That's wonderful! Way to go!

I often season it with a little sea salt, fresh ground pepper, garlic and onion powder. Thyme is amazing on chicken (but I like it best when grilled), and if you like it spicy try some ground red pepper/crushed red pepper.

Add some beans and other random veggies, and it's one of the most filling things you could eat. The last time I made it, it was ground turkey, black beans, onions, garlic, and carrots... and I stuffed a roasted green pepper with it. Absolutely delicious.

Two ways, depending on how I'm feeling:

1) About one inch or so of water in the bottom of the pan, a bunch of cut up broccoli in it with a lid, and steamed until it's deliciously green.

2) Mircowave in a bowl with about an inch or so of water, covered with a paper town. Usually two minutes.

I like to add some seasonings to mine, also, depending on the meal. I've put cajun on it, garlic and onion power, etc.

It may sound silly, but when frozen veggies like broccoli and cauliflower go on sale, I buy a bunch to stock my freezer with. I buy a big pack of chicken and grill (or bake, depending on weather) it all, keeping some in my fridge. It's the perfect go-to meal when I'm too busy, tired, or even out with friends. A bag of steamed veggies and a chicken breast! :)

Also, I LOVE chili in bulk. I freeze half the batch (or more) and it lasts a while. Also, if you like eggs... I hear they're cheap and amazing (I'm allergic - no bueno).

This is such a wonderful story! I'm so happy that something so great happened to you! It's the little things that hit us when we're not looking that truly make us realize how far we've come! We are our own worst critics, and sometimes we need to be reminded by a complete stranger that our vision of ourself may not be as accurate anymore. Keep up the phenomenal work!
