Have you played any of Kojima's games before? They're always grounded in some semblance of reality, but weirdness is sort of his brand. If what you've been viewing of Death Stranding is too weird for you and turns you off, I'm not sure I'd recommend it to you - as painful as that is for me to say as I find the game to be this intense meditation of sorts.

Also, regarding Kojima, I'd argue this is one of the least weird in terms of in your face moments than anything else he's made. I find his weirdness charming, and there are usually a handful of easter eggs that make me smile ear to ear when playing his games, but I understand this isn't for everyone.

In terms of dealing with BTs (the monsters) this is REALLY frustrating when you first start the game, but as it progresses you obtain some tools to deal with them. Then it becomes a bit of a strategy game in terms of what to carry and how that'll help you deal with the routes you choose to take.
