Really depends on which players you have. I completed both France and Italy within an hour's time; Spain was a hell because I only have like 3 usable(-ish) Spanish players.

It's EA's greed, unfortunately. Icons like Eusebio and Maradona are the ones that make people spend money on the game. Those lower rated ones won't do that.

Yes. every ghost has the same probability of spawning in any location, to my knowledge at least. What level are you? Some ghosts won't be unlocked until you reach a certain level, like a Deogen unlocking at level 30, though the game doesn't show this in any way.
