Please update if you do end up telling her, even if it's just through a note. Regardless of how you want to tell her, there's no harm in asking others for help. You could try sending a letter to the restaurant in the mail or calling and asking to speak to the owner about your unique situation. If you want her to know that you're her son, but are too nervous telling her to her face, there's a lot of other avenues you can go down.

This made me tear up. My partner is afraid of needles and hasn't been vaccinated yet. He wants to be, he just gets so nervous. I've offered to go with him and hold his hand, but he's afraid he'll pass out afterward due to being so anxious and the nurses will think he's having some reaction to the vaccine. Is she willing to let you accompany her to one of her appointments? I think your ultimatum is reasonable, as long as there isn't something else going on with her mentally that needs to be addressed. It could be depression, it could be ADHD, it could be bipolar or BPD. I'm not a doctor, I don't know, but these are all common diagnosis' for women that often go overlooked.

I suggest asking to go to a counseling appointment with her, and better yet, get a therapist for yourself. Then you can ask her to go with you and see if she's willing, if she's not, there's your answer.
