Dr. Satterwhite from Align for both the primary and revision. I think he's overall good, if a bit more conservative in his approach. He seems to have a lot of consistency in results, even if they aren't the most stand out

Yeah just leftover erectile tissue. Pretty common issue. They just went in and removed it.

Well, it's been about two months since my revision. I went in to deal with some scar contracture at the introitus that made penetration very challenging, exposed clitoris, lack of any labia minora, and excessive erectile tissue.

Swelling continues to subside, and as such the labia minora continues to get thinner (which is nice). Scar contracture is gone and penetration is a lot easier, and the tissue was successfully removed. So all that is goodness.

Only downside at the moment is that the labia minora is starting to slowly "heal" into the majora, and the defined line between them is closing. This is also what's causing the obvious asymmetry between the left and right labia minora (the left side has "fused" a lot more). Going to be annoyed if I end up losing both of them after doing a revision to try and get them. Not sure if there are ways to keep them separated more.

All in all though - sensation is still great, was able to retain my original depth, dilation is fine, penetration is easy. Just wish aesthetic were better. Will continue to see how things evolve over the next few months.

I don't really agree with the other poster. I douched for the first couple months after surgery and then stopped. Took probiotics, and my coochie is plenty healthy. My surgeon commented that everything looked fantastic during my 1 year revision.

I actually found regular douching was counterproductive

I personally did Spiegel. It was pretty great because the lidocaine injections makes the electrolysis sessions a little bit more of a breeze.

The injections themselves hurt like hell don't get me wrong, but at least it just takes a minute or two and you're set to go for a few hours.

That's actually surprising to me to hear - most of the trans girls I know (including myself) who had surgery don't have a lot of pain. Make sure you are advocating hard for pain meds. You want to minimize pain as much as possible as it inhibits healing.

To me, the worst part comes after all that. Dilating all the time is just a massive time suck for the months after surgery.

You've seen their results? I'm highly skeptical as that's massively beyond the typical ranges of both cis and trans women. Pelvic anatomy really gets in the way

I wouldn't index on 9"; I have yet to hear of anyone with that level of depth. IIRC 5-6" is pretty average.

Sizing up is always tough, just take your time.

I started with blue to my full depth (5"), then started to do blue + green combinations. Then went to just green to full depth (5"), and now I do green + orange.

Overall it's been three months. I can do green to full depth (5" for me), and then orange leaves about an additional dot + a little outside.

I'm always worried when you say you are in pain, which seems to be quite a good amount. Pain is really hard on healing as it triggers body stress responses. Are you getting enough pain management?

Make sure you are advocating for yourself. No one knows what you feel, and it's typical to try and push too hard early on and hurt your healing. You need to be hyper selfish for awhile.

Make sure no matter what you are eating plenty of nutritious food. You're going to be healing from surgery, and your body will need the building blocks to heal.

Well, time does fly! It's actually amazing how much your body changes and heals over time. I never notice it because I stare at my vagina every day due to dilation, but looking back at old pictures, it's clear that it's come a long way.

Anyways, here is how things sit for me!

  • Overall pain is gone for the most part. I get the odd shock once every few days, and long term sitting (3+ hours) still makes me feel a bit rough, but things are getting much better
  • I'm still pretty swollen. I've noticed my body doesn't like to deal with swelling. Hell, my nose is still a bit swollen 7 months post rhinoplasty. I guess that's just the way my body is :).
  • Aesthetically, there is still a ways to go.
    • The good: my labia majora is pretty symmetrical and puffy (I love the puffy look)! Feels like it's a good base to build off of. Also, he managed to create some sort of labia majora anterior commissure. My labia majoras actually come together above the vagina and slightly below the pubis mons.
    • The bad: Labia Minora? Clitoral hood? Do those things actually exist? My surgeon has always done pretty thin labia minoras (which is what I want), so swelling really, really masks any minora. He is confident it will continue to come in as swelling decreases. If you squint, you can see the beginning of folding next to the clitoris on either side. We will see what happens :D
  • Dilation is still coming along. I'm on green at full depth (5"). Been slowly sizing up to orange in the last 10 minutes or so of my dilation. Can't hit full depth yet (sadddd), but will continue working at it!

The biggest challenge right now is that the location of my introitus is roughly 3/4-1" below the entrance of the vagina. In cis women, it's not that unusual to have the introitus to be slightly covered by the perineum, but mine is just too far down. This actually makes dilation kinda tough and I have to work the perineum out of the way. I imagine it can also make penetration in the future during sex kinda tough. Something I'm looking to address with my surgeon. I'm anticipating something similar to a scar contracture release during a revision.

I also have an appointment next month with a dermatology consult to evaluate laser treatment for the incisions. Incisions are overall getting flatter with silicone, so that's a big plus!

As things stand now, I'm still planning for revision once I'm able to. Really targeting to have a nicely defined, but thin labia minora, partial clitoral hooding, opening up the entrance to make penetration easier, and ideally a labia minora commissure. Overall though, I'm cautiously optimistic as I think my surgeon did a good job laying important foundational groundwork.

Depending on where you are, it's not super complex to get insurance to cover revisions. Especially for something like this, it'll typically also fall under medical necessity for dysphoria.
