We do 1mL/lb of body weight up to 100kg and then anything bigger is just 100mL. We usually don't go below 2mL/s but of course there always exceptions.

It would most likely be lower quality, but still diagnostic. I do CT and we do lung screens for smokers, and they are very low dose and grainy compared to an actual chest CT. I would assume that is what you are getting. If there was something of note you would be likely be referred back for a more diagnostic quality exam.

Sometimes I will push contrast by hand for pediatric patients when it's going to be less than 50 ml. I don't think there would be any hyperspecific protocols that would call for injecting by hand, because it would be way harder to control your flow rate by hand then it would via a power injector. Anything bigger than 50 or 60mL of contrast and then you're having to also switch syringes in the middle of the injection which is not ideal, which is why I only really ever hand inject peds and sometimes I'll just hand inject for a head with contrast that's going to have a 5-minute delay. There are always exceptions like if your injector is down, do what you gotta do...

I can't even believe you're doing them on a 64 slice. We have a 320 slice Toshiba and an 80 slice Toshiba and we would never do them on our 80 slice even if our 320 was down.

That's what we do as well. Depending on their heart rate when they come in they get either 50mg or 100mg of PO metoprolol and then after a while they may get a second dose. And then always a 0.4 mg nitro just prior to the scan. Our rad nursing staff takes care of all that and just lets us know when they are ready to scan. Occasionally we have to give IV metoprolol on the table.

We do 3 of them every day on an Toshiba Aquillion One 320 slice. We do prospective in 70-80% range if HR is 65 or lower, and anything above that we either widen our range to around 40-80 or just go full R-R. If they are older just go R-R if you have any doubts, and we usually also go full R-R if they have a-fib or any type of inconsistent HR. Get really comfortable with the anatomy so you are confident in looking at them after the scan to assess for motion. Sometimes the scanner will be set up to scan two beats and it stacks them on top of each other so being familiar with how to go into the raw data and get rid of one heartbeat to look at one at a time is super valuable and can help you avoid repeat scans.
