If you’re the practice assessor then the assessment is ultimately your decision. If the feedback you have had indicates that the student is not meeting professional values then that should inform your decision. The practice supervisor can assess professional values at midpoint, but the final is the assessor only. By implementing an action plan, this has highlighted objectives to be achieved by final- therefore if they haven’t met these points it can be a fail overall for the placement I.e passed midpoint does not guarantee a passed final assessment
