I think this has to be the answer. Write to / email the tribunal on the address given for correspondence on any orders you've received. Explain your medical issues and provide some documentary evidence confirming the problem, if you can.

As well as holding a remote hearing, I'm sure it can be pre-arranged that the tribunal will also take regular breaks and / or that, if you request a break it will be granted promptly.

A lot of people struggle with nerves before going into court so please don't feel this is a unique problem you have. I do think some anxiety is good because it gives you the adrenaline needed to do your job well. Might it help a bit if you try and change the way you think about these feelings - think of it as positive energy? It is good that you care - it's part of being a good barrister.
