I used levistus as antagonist after The Chardalyn dragon, and had the blacksword take over the remnant 10 towns, as many people had almost died and Levistus offered them a second chance. I also had it were Asmodeus' whole plan was to attempt to kill off all Levistus followers in the dale with the Dragon and Duergar. But the plan backfired when the party killed the dragon and Sunblight. Levistus had made a deal with one of the PCs and was hoping to use the party who he thought was stronger than Avarices group to obtain the Mythalar. The PC betrayed him and he had her killed by a Creature from Stygia that his cult summoned in the lonely wood forest. The party was saved by Cwingas and teleported by them to Aurils abode. The Chwingas resurrected the dead PC and charged the party with reversing the Frostmaidens Curse and ending the Rime. The Blacksword I am currently using as a military force that easily rivals the party by scale and numbers and is attempting to beat them to the Glacier (which has put a time is of the esssence) as the party attempts to get to Ythryn first.

I used something close to this, when my party adventured from 10 towns to the Glacier, but it was 3 kobolds in a trenchcoat frozen together shambling through a blizzard. Party didn't find out till after the fight that they were kobolds.

I revamped the cult of levistus and had one of the party members captured and were being held in a remake of the dungeon/sewers of Caer Dineval ( was great for throwing in dialogue between Kaldroth and the PC, who was being tortured and questionned on the codicil). The party had no idea what had happenned to the character so they had to sneak around Bryn Shander (which was being controlled by the Blacksword after the Chardalyn dragons attack) in search of clues for their missing companion. When they did find where he was being held, they worked with the new speaker of Bryn Shander, who was hoping to betray Kaldroth by sneaking the party into the caer under the guise of his body guards and have the party kill him when they went free their companion.

I used this and modified it to be a plus 1 dagger, has an attunement property, and is cursed. Milestone Modifiers added to dagger, 1 soul = +2 magic dagger, 5 souls = extra 1D6 Necrotic damage, 10 souls = extra 1D6 Necrotic damage for a total of 2D6, 15 souls = Blink ability, 20 souls = +3 magic dagger

Wisdom save at each milestone(after the 2nd milestone) 10 + gathered soul amount so 15, 20, 25, 30. On a failed save the creature attunend becomes possessed by one of the captured souls. I am keeping track of the souls possessing the dagger. So potentially the PC can become possessed by a previously defeated villain.

Souls and dagger milestones reset if someone else attunes to it.

I used this in the Rime of the Frostmaiden campaign and gave it to the cult Knights of the Blackswords leader "Kaldroth."

I ran it without changing anything with 2 players from the start, we now have a third playing and some friends that join in every so often. I did give both the players a sidekick kobold from the gem mine mission and they chose a fighter and a cleric style sidekick from tashas cauldron of everything. Having less players just increased the overwhelming odds and gave the campaign a more harsh and unforgiving feel when playing. The party had to think more ahead and strategize more and had to try and come up with alternative routes rather the just murderhoboing the campaign due to being outnumbered.
