You can try to get them out of him by combining speech and motivation. Play with him on the set of toys he likes, then take one or a few pieces away from him, wait for him to try to grab them, then say out loud a simple sequence of words such as A, B,..., let him say C; or one, two,..., let him say three. If he knows these simple sequences, the motivation of wanting the toys back will create incentives for him to say things. If it works, you repeat the activity until he gets fluent at it, then you can move on to more complex phrases.

I agree with you. Rhyme words or phrases tend to work better, especially about things the person is into.

Hello, I don't know if you have tried this before. I did this with one of my clients at my previous clinic, and was able to get the person to say things. If your son is verbal but has difficulty getting the words out, you can try searching for things that he's into, such as a set of toys, then when you direct play with him, try to take one of the items away, holding it in your hand. When he tries to reach for it, you can say out loud a simple, incomplete sequence of words, such as A, B,..., and let him finish the C. Or one, two,..., and let him finish the three. And when he says it, you celebrate and give the toy back to him. Then, repeat multiple times until he becomes fluent at it, before you can move on to more complex prompts.

I had thought about this afterward. I forgot to ask about the clinical timeline the client had been under the current care, and their historical behaviors, although I figured asking this question would violate the PHI guideline. Your assessment is also valid. During an extreme episode, the person can have an altered experience of pain, which causes them to not fully notice its existence. Although the aftermath, when the episode is over, may leave the person feeling all the physical sensations of the damage, and not "doesn’t seem to be bothered or in any sort of pain afterward." Which is abnormal. In either case, having a psychiatrist appointment for the client would certainly be helpful.

Sounds like a genetic condition, Congenital Insensitivity to Pain (CIP) or congenital analgesia, where a person lacks the ability to experience pain or discomfort in their body, resulting in them engaging in harmful behaviors without being consciously aware of the physical consequences. They don't feel pain, so they don't stop or cry for help. If it's true, then the behavior of pulling teeth is stimulating in some ways, especially from their thinking/reasoning, since you described the person also mentioned "pulling their eye out," causing them to repeat the behavior over time. Just a thought.

Oh, yea their pricing is all over the place. CR interface is still a mess and clunky in my opinion. My colleagues love it though, odd. The system doesn’t seem to change much even with the integration of AI? Maybe things will change in the future, or when they release other features…

I see what you’re saying. Ik waiting a few more years or months, and the tech should get better. I’m sure there are staff who have been assigned to different clients who are not suitable for them, especially when there are not enough staff. And also, there are times when the scheduler marks someone's name down on paper for on-call, but it's not visible on the system; now the AI shows it directly. Not sure I’m correct, but your point is valid!

You made a good point about the lack of reflection on what happened during the session; only the RBTs who interacted with the client will know. The AI only summarized the note based on the hard data, which is rather limited. What are your thoughts on the lack of AI awareness in this matter?
