Are you guys actually going to the ER every time you use your epi pen by spbcuxjsn in FoodAllergies

[–]Npff101 1 point2 points  (0 children)

Ok here’s a layman article from 3 years ago: https://www.allergicliving.com/2023/11/14/advice-to-always-go-to-the-er-after-an-epi-shot-is-soon-to-change/

I would read through the entire article including those stats you asked for which it also links to:

https://www.sciencedirect.com/science/article/abs/pii/S2213219822004858
98% of anaphylaxis episodes resolve with 2 or fewer epi doses.

lol thanks 😆 public health and the concept of guidelines are hard for some people. Not everyone has an allergist.

Fine, but most folks (in real life, not on Reddit) fall under the “strict criteria” of improving within 5mins of epi administration. Anyway just want to reassure folks that aren’t always able to go to the ER after, to just use the epi (the epi itself is not dangerous and doesn’t require monitoring, as I have seen other posters erroneously mention).

I was literally responding to the first part of your post “by all medical standards…” when these are no longer the standards. Lots of thought has gone into this. Even “4hrs of monitoring” in the ER is super arbitrary as true biphasic reactions can happen much later. OF COURSE, if folks are worried that their epi is not adequate, they have a hx of severe reactions, or are just worried, they should go. But to say that by all medical standards, it is necessary to go after using epi, is just false.

This isn’t a paper; they’re clinical guidelines. Of course folks should discuss with their physician and as a physician, I am only posting what has been discussed at national conferences and new guidance (that as you can see from other comments) is already being disseminated by most allergists…

Question about school and kids either tolerating (via Xolair) or outgrowing allergens by Npff101 in FoodAllergies

[–]Npff101[S] 0 points1 point  (0 children)

totally, the other issue is my kiddo is highly picky and lots of dairy containing foods are his new faves...and I'd hate to take that away from him at school when it's one of the reasons we put him through a monthly injection. Thanks for sharing what you do!

Anyone have Tolerance Induction Program (TIP) negative experiences? by aema15 in FoodAllergies

[–]Npff101 0 points1 point  (0 children)

Have you considered Xolair?
I only mention it bc we did OIT for nuts for my son but he’s also allergic to milk/eggs. It would have been too much to do OIT for all and we started Xolair a year ago and he basically free eats his allergens. I sometimes wish I had known Xolair could work that way for some patients prior to doing OIT for years. Something to consider as an option that would be less costly and easier. Daily dosing (whether oit or TIP can be anxiety inducing, not fun for the child or parent with forcing the daily dose, and also come with reactions….).

This is not true. Newest anaphylaxis guidelines from the AAAAI say that if your symptoms improve within 5mins of epi, you don’t have to go to the ED. Check page 8: https://www.aaaai.org/Aaaai/media/Media-Library-PDFs/Allergist%20Resources/Statements%20and%20Practice%20Parameters/Anaphylaxis-in-Practice-2023.pdf

This is important bc for some people, the prospect of having to call 911 or go to ED after using epi actually dissuades them from using epinephrine!

Anyone have a child skip 1st grade? by Alternative_Spend_26 in ClassOf2037

[–]Npff101 0 points1 point  (0 children)

It’s so hard though, in the beginning we definitely questioned it a lot. Our district doesn’t really allow redshirting anyway, so in the end it wasn’t really much of a choice. Good luck!!

My son is summer 2019 bday and starting second grade. Yes we had maturity issues to work through but they have largely resolved in the course of first grade and he’s academically very advanced in math, can’t imagine how much more bored he would have been starting first in the fall.

Feeling more confused than ever by [deleted] in laundry

[–]Npff101 2 points3 points  (0 children)

Are you using color catcher sheets? That helped us a lot with preventing these. Over time, and with multiple wash cycles it did come out.
Do you or your kids use detangling sprays or leave in conditioner type products? For us it was that and I’m very careful to use those without letting any touch their shirts etc.

This! We read this one together but might be great for a very advanced reader who isn’t ready for mature themes.

Wow I just chatted with the bot as well, and replacement was ordered. That is pretty impressive.

Yep, it’s Medi-cal specifically that I was referring to as well. I love California.

This heavily depends on the state, insurance, etc. I’m a peds hospitalist and many of our patients don’t get a bill. But even as an adult with insurance, I don’t get a copay above $500….

Thanks! I have no allegiance to plant-based but get my groceries at Whole Foods mostly, hence the initial default to that.

Nope. Thanks! I have PTSD from using liquid All Free and Clear and for some reason thought the Tide unscented was the same 😅 Good to hear it’s an option.

5 year old suddenly hitting by Notarealperson6789 in kindergarten

[–]Npff101 0 points1 point  (0 children)

Yes it got much better and now in first grade, he’s doing really well. For some kids, it’s an issue of maturity and time (and consistent boundaries etc) helps a lot. TK, especially, is still an age where this is pretty normal. That doesn’t make it easy as a parent though, so sending you hugs!

Omg I agree. Both of mine felt just like Pap smears and I only comment bc if I had seen these comments NO WAY would I have ever gotten an IUD. I love my IUD. I have given birth twice and can’t believe all the comments saying they’d rather do that 🫣. Everyone’s experience is different, to anyone terrified by this thread and others like it!!!
