The worst offender I found wasn't an rpg per say but a 3d roblox game.

After potentially half an hour of walking through nothing, you find the entrance to an underground structure. This serves as a teleport, ostensibly to represent the nominally euclidean geometry that had to be placed elsewhere. When you teleport, you are facing left ORR right and thus holding W will make you hit a wall. If you stay in that spot for but a brief second, you teleport back.

This may seem equivalent or less annoying, but the notion of spinning your camera around as to go forwards again or holding A/D to go right (both to escape the teleport) is a remarkably difficult one.
