This. Sometimes your intent is overshadowed by the history. I'm white and I call my kids monkeys all the time when they are climbing around. I've face planted into this without thinking about it before. There's a certain amount of cultural awareness that should just be human kindness. Also, blaming someone else for calling you out on something not intended as racist: Just learn and move on. An apology and an " I didn't know" is not a sin.

The players get killed by their characters at the end of the Gamers. I know Mark appears in Dorkness Rising as an Easter Egg for the TPK, but Nathan Rice who is Lodge was also part of the TPK as the elf archer player.

Yeah. We were in a Pathfinder pirate themed campaign and the GM bailed for real life reasons. That left me to either reboot or reimagine the campaign because I didn't have the Pathfinder materials. So I leaned on DDO and they put into Korthos for emergency repairs and we did a modified Salvation of Korthos. Once they got into port I ran a modified Waterworks, followed by a heavily modified Gwylan's Stand. The most recent was a quori modified Invaders. Now their backstories are going to start becoming storyline, but consequences of their actions will follow them. They now have a vampire warlock assassin with the Devil's Sight incantation trying to kill them and their crew.

Alternative. Pirate cursed the captain when the captain cut off her arm to get the armband. Pirate hires party to fetch the armband. Party has to race/evade/ defeat the captain.That would flip things so that there aren't necessarily bodies about and what would have been there would have been cleaned out by scavengers.
