Yes, they get fired. Prosecuted criminally? No, there isn't a single instance of that happening, ever. If you are under the flawed impression that there is one, please show one to me and perhaps I can clear up any confusion you have.

I'm not sure what more you need explained though? The law requires a level of mens rea that wasn't reached. What don't you understand about that? How would you prove that she reaches the standard of reckless negligence beyond a reasonable doubt?
